Background and Aims: Poststreptococcal glomerulonephritis (PSGN) is the most common cause of acute glomerulonephritis (GN) in children.The diagnosis is based on clinical presentation that varies from asymptomatic microscopic hematuria to nephritic syndrome and the demonstration of recent group A beta-hemolytic streptococcus (GAS) infection.Method: We report the case of a 5 year-old-boy admitted to the Emergency Department (ER) for facial trauma with nasal edema.The next day, he started fever that lasted 5 days, with upper respiratory symptoms and headache.He returned to the ER 2 weeks later with new onset fever and macroscopic hematuria.Results: He had normal blood pressure and no edemas.The blood analyses showed leukocytosis, elevated C-reactive protein and anti-streptolysin O titer (5180U/mL), normal creatinine, low albumin and C3 (17mg/dL).Urinalysis proved hematuria and revealed nephrotic proteinuria. On day one of admission, he developed left periorbital edema and erythema, ocular pain, headache and vomiting. Given the clinical worsening, cerebral and orbital magnetic resonance was performed and revealed preseptal and postseptal cellulitis, and signs of sinusitis. Cerebrospinal fluid analysis showed no infection. Treatment with intravenous antibiotics (ceftriaxone and clindamycin) and corticosteroids was promptly initiated. The patient was discharged, after 10 days, with significant clinical improvement, no nephrotic proteinuria and no gross hematuria, on amoxicillin-clavulanate and corticosteroid weaning scheme.Conclusion: The association between sinus-related orbital cellulitis and PSGN as we present is rare, with only one case described in the literature to our knowledge.The simultaneous presentation makes it difficult to claim if these are two correlated or independent entities, given the pathogenesis of PSGN mediated by immune complexes.
