U.S. Army aviators have been authorized to wear contact lenses for aviation duties with special waiver since 1991. The authorized contact lens modality is the single-vision soft contact lens that corrects for 20/20 distance and near. As aviators become presbyopic, they often have to return to bifocal spectacle wear or readers over their distance contact lenses in order to complete their aviation duties. Spectacles introduce interface problems with head-mounted displays and night vision goggles. This pilot study evaluated the potential utility of multifocal contact lenses to provide adequate visual and flight performance for presbyopic aviators. Bifocal spectacles or readers provide better levels of visual performance than any of the three multifocal contact lenses evaluated. Flight performance in a UH-60 simulator under daytime conditions was not decremented with the multifocal contact lenses evaluated in this study; however, this study did not evaluate night flight, interface with head-mounted displays or night vision goggle flight performance. Studies specific to the Apache flight environment are being considered since visual performance remained within norms and Apache pilots are faced with greater interface problems with bifocal spectacles and head-mounted displays and protection mask configurations than pilots in other aviation platforms. Many Apache pilots have been permanently Wounded or converted to other platforms due to the onset of presbyopia and spectacle compatibility issues in the cockpit Multifocal contact lenses, when fit to specific parameters, may be an option under special circumstances where spectacles would lead to reduced performance.

Abstract: Major advances in contact lens design, along with improvements in measurement techniques of visual performance and subjective quality of vision, call for re-evaluation of the current presbyopic contact lens options. To assess the performance and patient satisfaction of monovision and bifocal contact lenses, we conducted a crossover study of 38 presbyopic patients. Patients were randomized into either bifocal (Bausch & Lomb SofLens Multifocal) or monovision (SofLens 59) for a month. Visual performance was measured with high- and low-contrast visual acuity at distance and near, time and number of errors on a computer-based letter-editing task, and near stereoacuity. Patients' satisfaction with the contact lenses were measured by the National Eye Institute Refractive Error Quality of Life Instrument (NEI-RQL) questionnaire, and by recording the patient's final lens preference. We found that patients maintained at least 20/20 binocular vision with both bifocal and monovision contact lenses under high-contrast conditions at distance and near. Under low-contrast conditions, patients lost less than a line of vision from the best spectacle correction to either bifocal or monovision contact lens correction at distance (ANOVA/Bonferroni p bifocal = 0.001, p monovision = 0.006). Myopia control is currently a topic of interest due to the increasing prevalence of myopia in the pediatric population of the United States and other areas of the world. Those who suffer from highly myopic refractive errors are predisposed to ocular complications due to exaggerated ocular axial elongation. Both orthokeratology and center-distance soft multifocal contact lenses have been proven through randomized clinical trials to be effective at slowing down axial elongation and minimizing myopic refractive error. There is supporting evidence that myopia control can be maximized by increasing the power and the area of the add within the optical design of a soft multifocal contact lens. This study aims to determine the maximum of these multifocal parameters in a center-distance and center-near soft multifocal contact lens design without causing a clinically meaningful reduction in objective and subjective visual performance. In this study, non-presbyopic subjects wore a total of thirteen different contact lenses; a single vision spherical control, six center-distance multifocals, and six center- near multifocals. For each multifocal design, there were two different add powers (+1.50 D and +2.50 D) and three different center optic zone sizes (1.5 mm, 2.0 mm, and 2.5 mm) assessed. Seventeen volunteer presbyopic aviators from Fort Rucker were fitted with five bifocal soft contact lens combinations and bifocal glasses. A four-phase investigation was conducted: a clinical phase involving the fitting of the bifocal contact lenses; a laboratory phase involving measurements of visual functions; a simulator phase involving measures of visual performance in simulated flight conditions; and an operational phase consisting of subjective responses regarding in-flight use of bifocal contact lenses. In general, vision in the best performing bifocal contact lenses typically was slightly reduced from vision in bifocal spectacles. The amount of the reduction, and whether it was clinically significant, depended on the refractive error of the subject (myopes generally performed better), the add group of the subject (low add group subjects performed better), and the type of bifocal contact lens. Aviators performed flight simulation maneuvers better in bifocal contact lenses than in bifocal glasses, and they evaluated their own ease of vision while performing aviation duties to be much easier in bifocal contact lenses than in bifocal glasses. Contact lenses and spectacles were compared to determine their relative merits and applications. The contact-lens types studied were: (1) a plstic, fluid, corneal-scleral design; (2) a plastic corneal design; (3) a plastic, ventilated, fluidless corneal-scleral design, and (4) a glass, ventilated fluidless cornea-sclreal design. The fluidless, ventilated lens proved superior to either the fluid or corneal type. The plastic lens appeared to offer greater protection of the eye since the lens is not easily broken. The major objection to plastic was attributed to poor wetting properties. In general, the contact lenses offered advantages in military field activities but were not superior in performance to spectacles in routine and clerical work. Contact lenses appeared particularly advantageous for use in inclement weather, arctic duty, swimming and violent activity. Corneal clouding was no longer a problem with the newer designs.
