HIV/AIDS levels are more than 50 per cent higher than predicted 10 years ago; by the end of 2000, more than 36 million people were living with HIV/AIDS. 5.3 million people were newly infected with HIV during the year 2000, with over half these new infections among young people. 95 per cent of all people infected with HIV live in developing countries, the majority living in Africa. Previously known as 'AIDS Update', this booklet is the tenth annual publication which provides an overview of the actions taken by the United Nations Population Fund (UNFPA) to prevent the spread of HIV infection and promote reproductive and sexual health concerns.

Previously known as AIDS Update, this is the 12th annual publication to provide information about actions taken by United Nations Population Fund (UNFPA) to prevent the spread of HIV infection and promote reproductive and sexual health concerns. In 2002, AIDS claimed 3.1 million lives. Several trends emerged during 2002, including a rise in infections amongst women, and the fact that the food crisis is compounding the epidemic in Southern Africa. Globally, five million people were newly infected in 2002, the equivalent of about 14,000 people each day, with more than 95 per cent of these in developing countries. It is estimated that 42 million people are now living with HIV/AIDS, with one third aged 15 to 24 years. AIDS is the leading cause of death in sub-Saharan Africa, and the fourth biggest killer worldwide.

In Africa, as in many parts of the world, adolescent reproductive health is a controversial issue for policy makers and programme planners. Adolescents are particularly vulnerable to HIV and AIDS and to a host of other problems such as sexually transmitted infection, unwanted pregnancy, unsafe abortions, sexual abuse, female genital mutilation and unsafe circumcision. Yet many countries do not have adolescent health policies in place and much remains to be done to ensure that adolescents can access appropriate sexual and reproductive health services. The authors of this volume present new perspectives and strategies to promote adolescent sexual and reproductive health. In particular, they make a unique attempt to bring together social and biomedical science and to disseminate concrete empirical evidence from existing programmes, carefully analysing what works and what does not at the local level.

The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

With the evolution in prevention, diagnosis, and treatment of HIV/AIDS, ending the infection as a public health threat worldwide has become a real possibility included within the United Nations Sustainable Development Goal Project 2030. However, some countries and even entire regions are not on track to reach this target due to increased new infections in young populations. Young women (15,Äì24¬†years) represent 48% of the new HIV cases globally. Research shows these women have significant unmet sexual and reproductive health (SRH) needs, with high rates of unplanned pregnancies, increased HIV perinatal transmission (HIV-PT) risk, and higher maternal morbidity and mortality. Granting access to SRH services based on rights for women living with HIV is a cost-effective alternative to reducing new infections in children by promoting respect for women,Äôs reproductive options. This chapter addresses the role of SRH services based on rights for women living with HIV within HIV-PT. It also summarizes the new ,ÄúConsolidated Guideline on Sexual and Reproductive Health and Rights of Women Living with HIV,Äù; designed by the World Health Organization as a global recommendation for SRH programs and services that promote gender equality and human rights for women living with HIV.

he starting point for this guideline is the point at which a woman has learnt that she is living with HIV and it therefore covers key issues for providing comprehensive sexual and reproductive health and rights-related services and support for women living with HIV. As women living with HIV face unique challenges and human rights violations related to their sexuality and reproduction within their families and communities as well as from the health-care institutions where they seek care particular emphasis is placed on the creation of an enabling environment to support more effective health interventions and better health outcomes. This guideline is meant to help countries to more effectively and efficiently plan develop and monitor programmes and services that promote gender equality and human rights and hence are more acceptable and appropriate for women living with HIV taking into account the national and local epidemiological context. It discusses implementation issues that health interventions and service delivery must address to achieve gender equality and support human rights.
