Buku ini merupakan salah satu tonggak perjuangan dalam upaya menguatkan layanan kesehatan primer di Indonesia yang selama ini dianggap sebagai layanan kesehatan yang ‘bukan prioritas’, oleh pemberi layanan kesehatan ‘kelas dua’, di tempat layanan kesehatan yang ‘kurang layak’. Tanpa layanan primer yang kuat maka kesehatan suatu bangsa akan mundur. Oleh karena itu, sebagian besar masalah kesehatan seharusnya dapat dicegah dan dikontrol dengan baik di layanan primer. Bila layanan primer kuat maka anggaran negara seharusnya dapat dihemat karena ditekankan pada upaya preventif dan bukan semata-mata kuratif. Buku ini memuat sekilas teori-teori tentang layanan kesehatan primer yang secara ideal diberikan oleh seorang ‘dokter layanan primer’ atau ‘dokter keluarga’ berdasarkan berbagai literatur di bidang kedokteran keluarga dan layanan primer, di tingkat nasional maupun internasional. Selain itu, pengalaman dari dokter-dokter praktik di layanan primer dalam merawat pasiennya juga diceritakan secara dialogis, termasuk upaya refleksi terhadap pengalaman praktik tersebut sehingga diharapkan dokter-dokter dapat belajar dari pengalaman ini, agar di kemudian hari dapat memberikan pelayanan kesehatan yang lebih baik lagi kepada pasien. Penguatan layanan kesehatan primer merupakan rekomendasi dari WHO, sejak Deklarasi Alma Ata 1978 sampai dengan sekarang ketika WHO mengeluarkan imbauan untuk menuju sistem jaminan sosial kesehatan dengan layanan primer sebagai tulang punggungnya (WHO, 2008). Indonesia merespons imbauan tersebut dengan adanya Sistem Jaminan Kesehatan Nasional melalui Badan Penyelenggara Jaminan Sosial yang masih terus disempurnakan. Bagi masyarakat umum, buku ini menjelaskan tentang paradigma pencegahan yang lebih baik dari mengobati, pentingnya keberlanjutan pelayanan sesuai dengan kebutuhan dan kepentingan pasien dari segala kelompok usia, sosial ekonomi, dan jenis kelamin, hubungan dokter-pasien yang intensif, baik di kala sehat maupun sakit, serta mengedepankan kolaborasi antartenaga kesehatan dalam upaya mewujudkan keselamatan pasien. “Kesehatan adalah hak asasi seluruh umat manusia.” (WHO) “Dokter yang baik, bukanlah yang paling pandai, tetapi yang mau belajar dari pengalamannya.” (Kata ‘belajar’ di sini mempunyai arti memahami keterbatasan diri, merenungkan, mengupayakan informasi berdasarkan bukti ilmiah terkini, dan berusaha berbuat yang lebih baik di kemudian hari.)

The concept of expressing acidity as the negative logarithm of the hydrogen ion concentration was defined and termed pH in the beginning of the 20th century. The general usefulness of the pH concept for life science was recognized and later gained importance to analytical research. Reports on results of pH measurements from living skin established the term acid mantle - the skin's own protective shield that maintains a naturally acid pH. It is invisible to the eye but crucial to the overall wellbeing of skin. Chronic alkalization can throw this acid mantle out of balance, leading to inflammation, dermatitis, and atopic skin diseases. It is therefore no surprise, that skin pH shifts have been observed in various skin pathologies. It is also obvious that the pH in topically applied preparations may play an important role. Optimal pH and buffer capacity within topical preparations not only support stability of active ingredients and auxiliary materials, but may also increase absorption of the non-ionized species of an acidic or a basic active ingredient. They may even open up opportunities to modify and "correct" skin pH and hence accelerate barrier recovery and maintain or enhance barrier integrity. Further efforts are needed to standardize and improve pH measurements in biological media or pharmaceutical/cosmetic vehicles to increase and ensure quality, comparability, and relevance of research data. In this volume, we present a unique collection of papers that address past, present and future issues of the pH of healthy and diseased skin. It is hoped that this collection will foster future efforts in clinical and experimental skin research.
