La douleur des sujets âgés est un symptôme très fréquent en médecine d’urgence. Elle est encore actuellement mal évaluée et insuffisamment prise en charge, particulièrement lorsqu'il existe des troubles de la communication. La diversité des étiologies et des tableaux cliniques, la variabilité d’expression de la douleur, la compréhension et l’attention que portent les soignants constituent autant de barrières à son évaluation et à sa prise en charge efficace. Aux urgences, la douleur est un signal d’alerte d’une affection pouvant potentiellement être grave, mais elle traduit également une souffrance qui mal évaluée peut avoir de graves répercussions. Une étude rétrospective, incluant 228 patients âgés de plus de 75 ans a été réalisée dans le service des urgences du centre hospitalier universitaire de Caen du 1er au 15 avril 2011. L’objectif était de réaliser un état des lieux de la prise en charge de la douleur aiguë par excès de nociception chez le sujet âgé de plus de 75 ans. Les résultats objectivent le manque d’évaluation de la douleur surtout lorsqu’il existe des troubles cognitifs mais aussi le déficit de la prise en charge antalgique, la difficulté à suivre les recommandations sur les antalgiques et la sous utilisation de la morphine. Le recueil de données rétrospectif a entraîné des difficultés par manque de traçabilité des informations. La mise en place de protocoles de soins, une meilleure formation et une plus grande sensibilisation du personnel soignant à la prise en charge de la douleur du sujet âgé amélioreraient ces résultats et ils pourraient être évalués par un autre état des lieux sous forme d’évaluation des pratiques professionnelles.

With the fall in mortality from infectious diseases, the· impact of childhood cancer in the Western World has increased to become the second commonest cause of death in the age group 1-14 years, being surpassed only by accidents. However, even in those countries in which paediatric cancer is attracting increasing interest, the disease is relatively uncommon, and the number of cases seen by any one physician, even in a large general hospital, is often limited to one or two a year. The widely held view by parents and even by many doctors that cancer in childhood is usually untreatable and inevitably fatal is no longer tenable. With improvement in the therapeutic response or in the actual survival rates of children with such lesions as Wilms' tumour, brain tumours, rhabdomyosarcomas, Ewing's sarcoma, retinoblastoma, lym phoma and even leukaemia, there is a real hope of achieving a substantial reduction in the mortality of childhood cancer.
