Operative Dictations in General and Vascular Surgery, Second Edition is intended to teach residents the principles of succinct and precise operative dictations for a wide spectrum of surgical procedures. Designed as a portable resource, the book provides typical dictations to guide the resident. Comprised of 226 procedures, this new edition comprises the majority of procedures commonly performed by general and vascular surgeons. For each procedure, a list of common indications is given. This list covers 95% of the situations in which a particular procedure will be used. A list of essential steps follows. These can be used to mentally rehearse the procedure before it is performed. The next heading, Note these technical variations, introduces a list that is intended to prompt the surgeon for particular things to note and dictate within the template. A list of possible complications that are typically associated with that particular surgical procedure follows. Finally, a template operative dictation is provided. Common opening and terminating sequences (for example, how to enter and close the abdomen) in all laparotomy dictations are included. The volume has two primary objectives. First, it is designed to serve as a source of model operative dictations that may be individualized and used as templates. Second, it is intended as an aide-memoir, giving the surgeon a short list of pertinent information about each procedure. Ample space is provided to allow each surgeon to add notes. By reviewing this introductory material before scrubbing on a procedure, the trainee surgeon will enter the operating room better prepared to participate actively and to learn as much as possible. At the end of training, this book, with the notes accumulated by the resident, will serve as an invaluable review resource analogous to the individualized notebooks many surgeons keep. Operative Dictations in General and Vascular Surgery, Second Edition is intended for practicing surgeons, who may modify each template to reflect their own individual practice. It also serves as a concise reminder of essential steps in those operations that may be only rarely performed.

The rising of the Lenten Moon signaled the beginning of a time of mournful respect for the death of Christ at the hands of the Romans. And two thousand years later the rising of the same full moon marked the beginning of a quest of discovery and accountability. To expose those who were entrusted with preserving life of committing acts of reckless abandonment. Deceit and arrogance were pieces of a failed orchestration that resulted in a tragic death. A missed diagnosis, the dodging of responsibility and the alchemy of leech saliva and snake venom were professional interventions that caused the instant clotting of the patients blood. Thirty one days of decisions and second-guessing tilled the soil of doubt as self inflicted guilt produced its fruit of reckoning. A reckoning that not only changed the lives of the guilty, but also provided insightful tutorials and saved my life in the process.

This edition of Bouncebacks! Emergency Medicine books examines the bounceback visit, putting the reader in the footsteps of the clinician as the patient deteriorates. The story pauses at important decision points as possible management strategies are reviewed in a literature-based fashion, followed by a revelation of the path chosen by the actual provider. As the patient’s course progresses, we reach additional decision points and address further clinical questions. In addition to our chapter authors, many with expertise in critical care emergency medicine, we have an all-star lineup of expert-whisperers: first an EM/critical care section editor (below) and second, EMCRIT.org’s double-boarded critical care guru, Scott Weingart. Their comments are inserted in grey boxes and are intended to give tips and tricks borne of experience and to provide context to the literature, "as if" they are standing over our shoulder and advising during a resuscitation. Additionally, there are three chapters where we address the medical-legal aspects of care. Enter Greg Henry MD, past president of The American College of Emergency Physicians (ACEP) and one of the most experienced physician medical-legal experts in the country. He opines on the approach that both the defense and plaintiff would likely pursue and, by extension, how we can make patients safer through our evaluation and documentation before there is an adverse outcome. Whereas, these chapters contain the actual documentation from the ED chart, a very few chapters have slight modifications to the final aspects of the return visit at decision point 3, to allow for exploration of different critical care scenarios. There is still plenty of commentary on the documentation, missed red-flags, and what could have been done to improve care at the initial visit. Our goal is to make each chapter challenging, dynamic, and realistic… to move us from “standard of care” to “excellence in care.” Book Review: "It's back, it's Bouncebacks! One of the mostly widely read and highly regarded education series in all of Emergency Medicine. Clinicians fear the medical malpractice landscape, but Mike Weinstock and the team give you all the tools you need to protect yourselves. Documentation people, documentation. Now with a critical care version. Read this book, sleep better at night, as simple as that! Love, love this series!" -- Mel Herbert, MD, MBBS, BMedSci, FACEP, FAAEMProfessor of Emergency Medicine at the Keck School of Medicine LAC + USC Medical Center, Owner and Editor of EM: RAP
