The purpose of this study was to examine prostate cancer screening practices among primary care physicians in the state of Missouri who routinely provide or recommend testing for their male patients. Prostate cancer screening rates were estimated using physician self-reported rates for six age-race categories. Preferred methods of screening for prostate cancer were evaluated and profiles of physician sociodemographic and practice-related factors associated with knowledge of prostate cancer facts were examined. Observational in design, the study was a descriptive/analytical cross-sectional mail survey that targeted the population of general practice, family medicine, and internal medicine physicians, who were actively practicing in Missouri. Surveys were sent to 3,271 eligible physicians in three successive mailings between July and October 1996. A total of 1,488 surveys were returned, resulting in a 46% participation rate. Prostate cancer screening rates differed by age groups, with lowest rates reported for men aged 40-49 years, and highest rates for men over the age of 50. No differences in screening rates were detected between racial groups, African American or Caucasian. The most commonly preferred screening method was the combined use of digital rectal examination (DRE) and serum prostate specific antigen testing (PSA). Adjusted odds ratios were derived by unconditional logistic regression to examine various factors in the likelihood to use this method. Patient age was the strongest predictor, followed by medical specialty, private practice, and physician involvement in direct patient care. Finally, physicians in private practice and those screening with DRE and PSA in combination, were more likely to score correctly on a knowledge subscale composed of prostate cancer facts. Explanations for the findings and further studies were recommended.
