International Symposium on the Treatment of Carcinoma of the Prostate, Berlin, November 13 to 15, 1969 presents a resolution to the doubts and uncertainty in the treatment of carcinoma and formulate a new policy for handling. Carcinoma of the prostate increasingly dominates the attention of urologists for both scientific and clinical reasons. The search for an explanation and the prediction of the variable behaviour of the malignant prostatic cell continues unabated. The search for more precise tumour staging and more effective treatment is equally vigorous. Editors Andrew Bruce and John Trachtenberg have assembled acknowledged leaders in prostate cancer to present those areas of direct interest to the clinician. There are a number of other topics that might have been considered but most of these, such as experimental tumour models or biochemical factors affecting cell growth, still lack immediate application for the clinician. Carcinoma of the prostate continues to have its highest incidence in the western world, and the difference in comparison with the incidence in the Far East appears to be real and not masked by diagnostic or other factors. The American Cancer Society anticipates that during 1995, 244000 new cases of cancer of the prostate will have been diagnosed in the United States, with 40400 deaths from the disease process. The rapid increase in the actual number of new cases in the United States is directly related to more regularized digital rectal examinations and the more frequent and widespread use of prostate-specific antigen (PSA) screening programs. As a conse quence, major controversies have emerged in how best to manage the patient with cancer of the prostate. These include the necessity of PSA screening programs, how to interpret PSA values, what imaging studies are most appropriate for definition of the true extent of the disease process, the value of pretreatment surgical staging of the disease process, and the value of different treatments. The latter encompass a wide range of options: observa tion, radical prostatectomy, the Walsh-type prostatectomy procedure, definitive external beam radiation therapy, local interstitial implantation using iodine-125, iridium-l92, or palladium-103, orchiectomy, cryosurgery, hyperthermia, and various antitestosterone agents such as estrogens or LH,RH agonists. The present volume deals with the major parameters of controversy with regard to cancer of the prostate and will enable the reader to acquire a better understanding of the disease and how best it might be managed to achieve an optimal outcome with a minimum of complications.

In February 1980 the Royal College of Radiologists, London, held its Second Symposium on Clinical Oncology. The subject was Prostatic Cancer and this publication is based on the presen tations at this meeting by leading authorities in the United Kingdom. The principal objective of these Symposia is to promote multi disciplinary collaboration for the benefit of cancer patients. Awareness and understanding of advances in a broad range of subjects are essential to ensure the timely and effective appli cation of new techniques in the prevention and treatment of all forms of cancer. Communication of this kind may also stimulate the creation of new ideas that may prove to be of more fundamental relevance to cancer research. Prostatic cancer has been afforded too little attention in the past, and its importance in Clinical Oncology has not been adequately recognised. And yet over the last 10 years consid erable progress has been recorded in our understanding of this disease and in its assessment and management. These achieve ments are well documented in this collection of papers and the outstanding problems are also clearly indicated. It is acknowl edged that the early diagnosis of prostatic cancer presents a continuing challenge, associated with the uncertainties that concern the clinical significance of 'latent' cancer of the prostate. The accurate assessment of the incidence and preva lence of the disease is extremely difficult and together these problems complicate the evaluation of the management of early or localised prostatic cancer.

It is uncommon for scientist, physician, and surgeon to meet with great regularity or for prolonged periods of time and it must be exceedingly rare for such people to immerse themselves in each other's work and company for aperiod of almost two weeks. For this to occur in a situation of total isolation such as that provided by the marvellous Ettore Majorana Centre in Erice, Sicily must be unique. The fact that differences of opinion remain will be evident to all who read the book, as will the wealth of scientific and clinical work being undertaken within and beyond the NATO countries.
