El objetivo de este protocolo es facilitar la evaluación de los establecimientos de salud de América Latina y el Caribe en lo relativo a agua potable, alcantarillado, saneamiento, higiene, residuos sólidos y limpieza. El protocolo sigue dos herramientas principales: la determinación de una muestra significativa, respaldada estadísticamente con los datos de cada país, y un cuestionario compuesto por siete módulos: agua potable; saneamiento; higiene; residuos; limpieza hospitalaria; energía y medio ambiente; y gestión y fuerza laboral. El éxito del protocolo depende de que se respeten los procedimientos y la metodología propuesta, que se puede desarrollar en un período de tres a cuatro meses, según el tamaño de la muestra de establecimientos de salud y las facilidades que se les brinde para levantar la información. La metodología del protocolo comprende varias etapas: toma de decisiones, determinación de la muestra, coordinación institucional, capacitación del personal encuestador, levantamiento de información, procesamiento de información y análisis de la información y presentación de resultados.

Este informe se basa en la aplicación de un protocolo diseñado conforme a las recomendaciones de la Organización Mundial de la Salud sobre los servicios de agua, saneamiento e higiene en establecimientos de salud, cuya metodología permite definir las características de dichos servicios y evaluarlos. Los resultados abarcan los muestreos realizados en los establecimientos de salud de los siguientes siete países: Bolivia (Estado Plurinacional de), Guatemala, Honduras, México, Panamá, Paraguay y Perú, con una muestra representativa y un valor de confianza de 95%. En los componentes estudiados se incluyeron los parámetros básicos del Programa Conjunto OMS/UNICEF de Monitoreo del Abastecimiento del Agua, el Saneamiento y la Higiene, a fin de determinar la gestión segura del agua y el saneamiento, y se estimaron indicadores avanzados que proporcionan una visión más detallada de las características de estos servicios en los establecimientos de salud. Entre otros resultados, se destaca que 17% de los establecimientos de salud de los 7 países analizados todavía no cuente con abastecimiento de agua conectado a la red pública. Otro aspecto relevante es que 54% de estas instituciones presenta problemas de calidad de agua, 12% no dispone de baños operativos para pacientes y 40% carece de jabón para lavarse las manos. Además, en 17% de los establecimientos de salud no se clasifican los residuos sólidos generados, mientras que en 15% dichos residuos no se tratan. Por último, 92,5% presenta problemas de vectores, roedores y fauna nociva en sus instalaciones.

In many ways the power of the social protection floor lies in its simplicity. The floor is based on the idea that everyone should enjoy at least basic income security sufficient to live, guaranteed through transfers in cash or in kind, such as pensions for the elderly and persons with disabilities, child benefits, income support benefits and/or employment guarantees and services for the unemployed and working poor. Together, in cash and in kind transfers should ensure that everyone has access to essential goods and services, including essential health services, primary education, housing, water and sanitation. This report, prepared under the guidance of Ms Michelle Bachelet and members of the Advisory Group, shows that the extension of social protection, drawing on social protection floors, can play a pivotal role in relieving people of poverty and deprivation. It can in addition help people adapt their skills to overcome the constraints that block their full participation in a changing economic and social environment, contributing to improved human capital development and stimulating greater productive activity. The report also shows how social protection has helped to stabilize aggregate demand in times of crisis and to increase resilience against economic shocks, contributing to accelerate recovery towards more inclusive and sustainable development paths.
