The authors of this volume point out that what is ordinarily termed the psychiatric hospital's "social structure" is principally derived from three sources: the number and kinds of professionals who work there; the treatment ideologies and professional identities of these professionals; and the relationships of the institution and its professionals to outside communities, both professional and lay. They describe hospitals as sites where ideological battles characterizing the mental health arena are being fought, implemented, critiqued, modified, and transformed. This classic monograph in medical sociology was originally published in the 1960s. The period studied was 1958 through 1963, when somatic and psychotherapeutic ideologies were flourishing—as now—and milieutherapy was just emerging. The research team was multidisciplinary: three sociologists, one psychologist, and one psychiatrist. Three distinct psychiatric environments were researched: two at the Chicago State Hospital—"chronic services" and "treatment services"—and one at a private hospital. What evolved were thoughtful comparative analyses of hospitals, wards, professionals, ideological positions, careers, and organizational and situational placements.

When first published in 1970, Stations of the Lost won the C. Wright Mills Award for Best Book in the Area of Social Problems. The study considers the Skid Row alcoholic from two points of view, that of the alcoholic himself and that of the agents of social control who treat him. A major discovery of Wiseman's research was that Skid Row men spend only about one third of the year on Skid Row. The rest of the time is spent "making the loop"—going from Skid Row to city jail, to county jail, to the state mental hospital, to the missions, and back to Skid Row. While these facilities are designed to handle or rehabilitate Skid Row men, they are actually used by these men as a means of survival.

This classic study deals with social control in advanced industrial society, especially the United States, and particularly the half-century after World War I. The United States is representative of Western advanced industrial nations that have been faced with marked strain in their political institutions. These nation-states have been experiencing a decline in popular confidence and distrust of the political process, an absence of decisive legislative majorities, and an increased inability to govern effectively, that is, to balance and to contain competing interest group demands and resolve political conflicts.Janowitz uses the sociological idea of social control to explore the sources of these political dilemmas. Social control does not imply coercion or the repression of the individual by societal institutions. Social control is, rather, the face of coercive control. It refers to the capacity of a social group, including a whole society, to regulate itself. Self-regulation implies a set of higher moral principles beyond those of self-interest.Since the end of World War II, the expanded scope of empirical research has profoundly transformed the sociological discipline. The repeated efforts to achieve a theoretical reformulation have left a positive residue, but there have been no new conceptual breakthroughs that are compelling. This book is a concerted and detailed effort organize and to make sense out of the vastly increased body of empirical research.
