This book investigates what was distinctive about the predisposition to psychosis Freud posited in Daniel Paul Schreber, a presiding judge in Saxony's highest court. It argues that Freud's 1911 Schreber text reversed the order of priority in late nineteenth-century conceptions of the disposing causes of psychosis - the objective-biological and subjective-biographical - to privilege subjective disposition to psychosis, but without returning to the paradigms of early nineteenth-century Romantic psychiatry and without obviating the legitimate claims of biological psychiatry in relation to hereditary disposition. While Schreber is the book's reference point, this is not a general treatment of Schreber, or of Freud's reading of the Schreber case. It focuses rather on what was new in Freud's thinking on the disposition to psychosis, what he learned from his psychiatrist contemporaries and what he did not, and whether or not psychoanalysts have fully received his aetiology.

Encephalitis lethargica (‘sleeping sickness’) was a mysterious disorder that swept the world in the decade following the First World War, before disappearing without its cause having been identified. Around 85% of its victims, predominantly children, adolescents and younger adults, survived the acute disorder, but most developed severe neurological syndromes, particularly severe post-encephalitic parkinsonism and other severe motor abnormalities, that incapacitated them for the remainder of their lives. Despite its brief history, encephalitis lethargica played a major role in a variety medical discussions between the two World Wars, as this epitome of neuropsychiatric disease – attacking both motor and mental functions – appeared just as the separation of neurology and psychiatry had reached a critical point. Encephalitis lethargica sufferers presented an unprecedented combination of neurologic and psychiatric symptoms – including previously puzzling phenomena primarily associated with schizophrenia and hysteria, as well as behavioral changes and attention deficit disorders in children – that not only underscored the unity of mind and movement in the CNS, but also illuminated the critical role played by subcortical structures in consciousness and other higher mental functions that had formerly been associated with the soul and more recently presumed to be localized to the human cerebral cortex. Encephalitis lethargica exerted a greater influence on clinical and theoretic neuroscientific thought between the two World Wars than any other single disorder and had an enduring impact upon neurology and psychiatry. This book will be of interest to an educated audience active or interested in clinical (neurology, psychiatry, psychology) or laboratory neuroscience, particularly those interested in neuropsychiatry, as well as to those interested in the history of the biomedical sciences.

Die Entwicklnng unseres Wissens auf dem Gebiete der klinischen Psychiatrie in den letzten 3 Jahrzehnten spiegelt sich im Umfang und Inhalt der Beiträge des vorliegenden Bandes. Bei den endogenen Psychosen, Neurosen und abnormen Reaktionen standen Bemühungen um die Therapie im Vordergrund. Der Nach druck des Forschers und Klinikers hat sich auf die Behandlung der Schizophrenien, der manisch-depressiven Erkrankungen, der Epilepsien und auf vordem weniger beachtete Gebiete verlegt: Suchten, organische Psychosen und Hirnerkrankungen und besonders die Epilepsien sind in der heutigen Psychiatrie eingehender studiert worden als je zuvor. Die klinische Symptomatologie wurde weniger gefördert und bearbeitet. Die Gründe für das Abflauen des Interesses an Erscheinungsweise, Abgrenzung und Verlaufsform sind nicht leicht zu finden. Es mag sein, daß mit den Definitionen KURT SCHNEIDERS, die in fast jeder Arbeit zitiert werden, die Mög lichkeiten der Weiterentwicklung zunächst einmal erschöpft waren. Diese an sich wichtigen Definitionen haben die Hypothesenbildung und den Beobachtungs rahmen in der deutschen Psychiatrie zunächst auf praktisch-diagnostische Fragen beschränkt, neben denen eine reine Psychopathologie oder spezielle nosologische Abgrenzungen fortgeführt wurden. Versuche, diesen Rahmen durch psycho analytische und existentialistische Interpretationen zu sprengen und auch in der europäischen Psychiatrie zu einer "dynamischen" Auffassung der Psychosen zu gelangen, werden im 1. Band eine Darstellung-finden. Seit Beginn der dreißiger Jahre hat sich in unerwarteter Weise eine wachsende Welle von Behandlungsversuchen auf die somatische Therapie der endogenen Psychosen konzentriert und hat die geduldige, resignierte Anstaltsatmosphäre verjagt.
