Belastende Lebensereignisse oder psychische Traumata? Welche Behandlung ist am besten geeignet? Die Diagnose und Therapie von Traumafolgestörungen sind anspruchsvoll und komplex. Psychotherapeut:innen sehen sich zunehmend mit Patient:innen konfrontiert, bei denen eine Posttraumatische Belastungsstörung zumindest als Komorbidität vermutet wird, oft ohne klare Vorstellungen darüber, was eine traumafokussierte Behandlung tatsächlich beinhalten soll. Umso wichtiger ist die sorgfältige Unterscheidung zwischen der Verarbeitung von unspezifischen belastenden Lebensereignissen und psychischen Traumatisierungen. Das vorliegende Zusatzmodul soll als Ergänzung zum OPD-3-Manual dazu beitragen, diese Differenzierung zu schärfen, da dies erhebliche Auswirkungen auf die Auswahl der individuell geeigneten Psychotherapie hat. Das Modul erweitert darüber hinaus die Beurteilung der psychodynamischen Grundlagen um die Auswirkungen traumatischer Ereignisse auf die Psychodynamik. Es berücksichtigt, wie individuelle psychodynamische Voraussetzungen die Wahrnehmung und Verarbeitung belastender Lebensereignisse beeinflussen und so vor einer Folgestörung schützen oder die Entwicklung dieser begünstigen können. Der diagnostische Prozess erfolgt in zwei Stufen: zunächst nach OPD-3 die Erfassung der psychodynamischen Ausgangssituation vor dem traumatischen Ereignis, gefolgt von einem zweiten diagnostischen Schritt, die Auswirkungen des Ereignisses mithilfe dieses Moduls zu operationalisieren. Dies ermöglicht die Unterscheidung zwischen spezifischer Symptomatik und Psychodynamik, die Identifizierung von Ressourcen und vulnerablen Faktoren sowie die frühzeitige Erkennung von Therapiehemmnissen. Prägnante Fallbeispiele verdeutlichen die Potenziale traumafokussierender Psychotherapien, aber auch die Notwendigkeit, bei Patient:innen mit gering integrierter psychischer Struktur sorgfältig zu prüfen, ob eine traumafokussierte Behandlung erst nach Stabilisierung indiziert ist. Operationalized Psychodynamic Diagnosis (OPD) is a form of multiaxial diagnostic and classification system based on psychodynamic principles, analogous to those based on other principles such as DSM-IV and ICD-10. The OPD is based on five axes: I = experience of illness and prerequisites for treatment, II = interpersonal relations, III = conflict, IV = structure, and V = mental and psychosomatic disorders (in line with Chapter V (F) of the ICD-10). After an initial interview lasting 1–2 hours, the clinician (or researcher) can evaluate the patient's psychodynamics according to these axes and enter them in the checklists and evaluation forms provided. The new version, OPD-2, has been developed from a purely diagnostic system to include a set of tools and procedures for treatment planning and for measuring change, as well as for determining the appropriate main focuses of treatment and developing appropriate treatment strategies.
