The World Health Organization recognizes that motor vehicle crashes and their impacts have become a serious public health crisis, with injury rates expected to escalate globally over the next 30 years. The aim of the current thesis was to investigate the development and maintenance of posttraumatic stress disorder (PTSD), travel phobias and depression after road traffic accidents (RTA), with a specific focus on cognitive factors. Two samples of injured RTA survivors were tested, the first in a cross-sectional (n = 101) and the second in a prospective longitudinal design (n = 147). -- Cognitive factors predicted PTSD, travel phobias and depression better than established predictors identified in past research. In addition, variables derived from cognitive models were found to be disorder-specific (Chapter 3). -- Chapter 4 investigated characteristics of the accident memory. RTA survivors with PTSD showed higher disorganisation in voluntarily retrieved accident narratives than participants without PTSD. However, this effect was not specific for the recall of the accident. Results of two implicit memory tasks provided partial support for the hypothesis of heightened perceptual priming for accident-related stimuli in PTSD. -- The role of rumination in the maintenance of PTSD and depression was investigated in Chapter 5. Rumination significantly predicted both disorders, was related to perseverance in a rumination task and was characterised by reduced concreteness of thinking. -- Finally, Chapter 6 examined psychobiological predictors for emotional disorders following RTA. Low heart rate as well as low basal cortisol levels assessed a few hours after the accident predicted emotional problems at follow-up. Complex relationships between the disorders and heart rate reactivity to accident imagery as well as basal cortisol levels at two weeks and six months were found. -- The findings of the current thesis support the view that cognitive factors are involved in the development and maintenance of PTSD, travel phobias and depression after RTA.
