" KARL JASPERS Since the publication of KARL JASPERS' General Psychopathology in 1913 at least, it has become obvious that a psychopathologist cannot do without philosophical reflection. If he wants to say anything about disorders of perception and thought, or of the experiencing I, to name the subjects of this book, he must know those disorders and the problems related to them. The phenomena and problems in question are not at all simply empirical (which is to say that they concern the structure and function of the organs of sense or the central nervous system), but conceptual. We have already formed some notions and prejudices concerning those subjects - perception, e. g. , is considered to be "a projection from the outside into the inward", thinking "a kind of inward talk", the I refers to "reflec ting upon me by myself', etc. Unless we think about these ideas critically, we shall fail in describing disorders of experience, thought, and action.

This book presents an innovative approach to clinical assessment in psychiatry based on a number of psychopathological dimensions with a presumed underlying pathophysiology, that are related to fundamental phenomenological aspects and lie on a continuum from normality to pathology. It is described how the evaluation of these dimensions with a specific, validated rapid assessment instrument could easily integrate and enrich the classical diagnostic DSM-5 or ICD-10 assessment. The supplemental use of this dimensional approach can better capture the complexity underlying current categories of mental illness. The findings from a large patient sample suggest how this assessment could give a first glance at how variable and multifaceted the psychopathological components within a single diagnostic category can be, and thereby optimise diagnosis and treatment choices. Being short and easy to complete, this dimensional assessment can be done in a busy clinical setting, during an ordinary psychiatric visit, and in an acute clinical context, with limited effort by a minimally trained clinician. The first edition of Comprehensive Handbook of Psychopathology was published in 1984, al most a decade ago. In the interim there has been an explosion of information in psychopathology. Proliferation of knowledge has included a widening base of research data and changing or new concepts and theories regarding classification, measurement methods, and etiology of abnormal behaviors and mental disorders. It has been an active and productive period for biological and behavioral scientists and clinicians, particularly in terms of changing notions of the complex interaction of environmental and biological factors in many disorders. For example, with the classic disorders-such as anxiety and dissociative disorders-our understanding, while far from perfect, has been greatly enhanced in recent years. Whereas there was almost a vacuum of empirical knowledge ten years ago about the personality disorders, concentrated efforts have been undertaken to investigate classification, comorbidities, and expression of the personality disorders, and variants in normal personality traits. In addition, scientific advances in the fields of behavioral medicine, health psychology, and neuropsychology have greatly contributed to our knowledge of psychopathology and the interplay of psychobiological factors. It is now commonly acknowledged that psychopathology is not limited to the traditional mental illness categories; it also plays a significant role in many physical illnesses, such as cancer and AIDS. With these developments, it became clear that the first edition of this handbook was outdated and that a revision was needed.
