" KARL JASPERS Since the publication of KARL JASPERS' General Psychopathology in 1913 at least, it has become obvious that a psychopathologist cannot do without philosophical reflection. If he wants to say anything about disorders of perception and thought, or of the experiencing I, to name the subjects of this book, he must know those disorders and the problems related to them. The phenomena and problems in question are not at all simply empirical (which is to say that they concern the structure and function of the organs of sense or the central nervous system), but conceptual. We have already formed some notions and prejudices concerning those subjects - perception, e. g. , is considered to be "a projection from the outside into the inward", thinking "a kind of inward talk", the I refers to "reflec ting upon me by myself', etc. Unless we think about these ideas critically, we shall fail in describing disorders of experience, thought, and action.

Nach wie vor besteht das Vorurteil, dass eine komplexe Psychotherapie, bei der auch an den Selbstkonzepten gearbeitet wird oder Emotionen fokussiert werden, bei Psychosen nicht indiziert ist. In diesem Buch wird mit diesem Vorurteil aufgeräumt. Der Band beschreibt das Vorgehen bei der kognitiv-verhaltenstherapeutischen Behandlung von Patienten mit einer psychotischen Störung in der ambulanten Praxis. Er liefert Antworten auf häufig gestellte Fragen wie z. B.: Was mache ich, wenn der Patient die Diagnose ablehnt oder wenn er abrupt seine Medikamente absetzt? Wie gehe ich mit akuten Krisen und formalen Denkstörungen um? Anhand zahlreicher Fallbeispiele liefert der Band zunächst eine Beschreibung der Störung und das für die Therapie nötige Hintergrundwissen. Neben der bekannten biologischen Vulnerabilität wird auch auf psychosoziale Risikofaktoren, wie z. B. Probleme in der Stress- und Emotionsregulation, bestimmte Urteilsverzerrungen sowie familiäre Interaktionsmuster, eingegangen, die bei der Entstehung von Psychosen eine wichtige Rolle spielen. Den Schwerpunkt des Bandes bildet die Beschreibung des Vorgehens in der kognitiven Verhaltenstherapie von Psychosen sowie bei verhaltenstherapeutischen Familieninterventionen. Hierzu wird u. a. auf die Erarbeitung des individuellen Störungsmodells sowie die Arbeit an auslösenden und aufrechterhaltenden Faktoren eingegangen. Zudem werden Interventionen für den Umgang mit Wahn, Halluzinationen und der Negativsymptomatik vorgestellt sowie das Vorgehen beim Kommunikations- und Problemlösetraining.
