Abstract: Abstract of thesis entitled "Psychophysiological Effects of Psychosocial Interventions - An Example of Breast Cancer Patients in Hong Kong" Submitted by Ho Tin Hung, Rainbow for the degree of Doctor of Philosophy at The University of Hong Kong in January 2005 Bringing physiology into psychosocial intervention outcome studies in psycho-oncology has been drawing increasing attention in recent years, as it provides an integrative perspective for investigating the mediators of cancer progression. Following this direction, the present study adopted a longitudinal randomized controlled design to investigate the psychophysiological outcomes of different psychosocial interventions for breast cancer patients. Seventy-six breast cancer patients in Hong Kong were recruited and randomly assigned into groups that received three different intervention approaches, namely, Body-Mind-Spirit (BMS), Supportive-Expressive (SE), and Social Support Self-help (SS) approaches; a no-intervention group was used as control. Psychological and physiological data were collected at baseline, 4 months and 8 months. Outcome measures included the General Health Questionnaires, Perceived Stress Scale, Hospital Anxiety and Depression Scale, Emotional Control Scale, MiniMAC, and Yale Social Support index. Salivary cortisol was used as the physiological stress markers. Overall, results indicated that the BMS intervention produced the greatest and the most sustained effects. It reduced psychological distress (p = 0.01) and enhanced positive social support after 4 months (p=0.02). Emotional control (p=0.02) and negative mental adjustment (p=0.04) were reduced after 8 months. Total salivary cortisol was lowered after 8 months (p=0.03); salivary cortisol concentration was lowered after 4 months (p=0.03) and 8 months (p=0.02). Besides feeling supported, participant reported that BMS intervention equipped them with sustainable knowledge and insights for meaning reconstruction from their cancer experience. Participants in SE groups showed improvements in psychological distress, emotional control and mental adjustment, but the changes were not statistically significant. Most subjects viewed the SE treatment as helpful; in particular, professional advice was viewed as useful and self-hypnosis was regarded as effective in reducing stress. Overall, participants in the SS groups seemed less likely to benefit from the intervention. Negative mental adjustment increased after 4 months (p=0.004). Only half of subjects evaluated the group as effective, five (31%) of them stated that they derived no benefit at all from participation. The no intervention control group indicated a reduction in social support network size after 4 month (p=0.04) and 8 months (p=0.01); and reduced positive support after 8 months (p=0.04). These outcomes suggest that social gathering may not be sufficient to produce psychophysiological improvement. Active professional intervention, providing a supporting context, encouraging expression, providing relevant information and teaching stress management techniques are more likely to yield therapeutic effects. In particular, attending to the spiritual dimension, which has been largely ignored in psychosocial interventions, contributes to positive outcomes. In Meeting Psychosocial Needs of Women with Breast Cancer, the National Cancer Policy Board of the Institute of Medicine examines the psychosocial consequences of the cancer experience. The book focuses specifically on breast cancer in women because this group has the largest survivor population (over 2 million) and this disease is the most extensively studied cancer from the standpoint of psychosocial effects. The book characterizes the psychosocial consequences of a diagnosis of breast cancer, describes psychosocial services and how they are delivered, and evaluates their effectiveness. It assesses the status of professional education and training and applied clinical and health services research and proposes policies to improve the quality of care and quality of life for women with breast cancer and their families. Because cancer of the breast is likely a good model for cancer at other sites, recommendations for this cancer should be applicable to the psychosocial care provided generally to individuals with cancer. For breast cancer, and indeed probably for any cancer, the report finds that psychosocial services can provide significant benefits in quality of life and success in coping with serious and life-threatening disease for patients and their families.

Cancer care today often provides state-of-the-science biomedical treatment, but fails to address the psychological and social (psychosocial) problems associated with the illness. This failure can compromise the effectiveness of health care and thereby adversely affect the health of cancer patients. Psychological and social problems created or exacerbated by cancer-including depression and other emotional problems; lack of information or skills needed to manage the illness; lack of transportation or other resources; and disruptions in work, school, and family life-cause additional suffering, weaken adherence to prescribed treatments, and threaten patients' return to health. Today, it is not possible to deliver high-quality cancer care without using existing approaches, tools, and resources to address patients' psychosocial health needs. All patients with cancer and their families should expect and receive cancer care that ensures the provision of appropriate psychosocial health services. Cancer Care for the Whole Patient recommends actions that oncology providers, health policy makers, educators, health insurers, health planners, researchers and research sponsors, and consumer advocates should undertake to ensure that this standard is met.
