One of the most important questions of our previous common volumes about affective, schizoaffective, and schizophrenic disorders was the question of what connects and what separates psychotic disorders (Marneros and Tsuang, Schizo affective Psychoses, Springer-Verlag, 1986; Marneros and Tsuang Affective and Schizoaffective Disorders, Springer-Verlag, 1990; Marneros, Andreasen, and Tsuang, Negative and Positive Schizophrenia, Springer-Verlag 1993). The boundaries between various psychotic disorders are not always clearly defined. Some groups of psychotic disorders, such as schizoaffective disorders and all the other "atypical" psychoses, occupy a position between "typical" mental disorders, such as schizophrenia, and affective disorders. The question is: Do psychotic disorders form a continuum, or are they, despite their unclear boundaries, distinct entities? On what basis should we assume there is a continuity of psychotic disorders? Solely symptomatology? Or perhaps also a continuity in genetic predispositions? Or in biological, pharmacological, and other dimensions? Is the old idea of "Einheitspsychose" (unitary psychosis) really always wrong? The contributions contained in this new volume cannot provide a definite answer to the above questions. Impairments in mentalizing - the capacity to utilize mental state information to understand oneself and others - have consistently been identified across the developmental continuum of psychosis expression, from the premorbid and prodromal stages to its clinical forms. Mentalizing difficulties in psychosis have been investigated using an array of different methodologies, including novel experimental tasks, narrative assessments, self-report measures, as well as neuroscientific and computational methods. These studies have primarily examined how mentalizing disturbances relate to symptom dimensions and functional outcomes in clinical samples, as well as the transition to clinical psychosis among those who are at increased risk. More recently, clinical adaptations of mentalization-based treatments (MBT) and other psychotherapeutic approaches with a focus on supporting people suffering with psychosis reflect on their own and others’ mental states, such as Metacognitive Reflection and Insight Therapy (MERIT), have been reported in the literature.

"Background: While some symptoms of schizophrenia and psychotic disorders (e.g., hallucinations and delusions) can be alleviated with proper treatment, most patients are left with social and functional limitations. Negative symptoms, cognitive impairment and insight deficits represent key therapeutic targets given their strong predictive power of psychotic conversion and relapse, and their robust association with functioning. These three dimensions still represent unmet therapeutic needs and efforts must be pursued to develop more efficient treatments. The heterogeneous and changing nature of the illness represents a significant barrier to improve current treatments. One source of such variation has to do with the different stages of the illness. According to the clinical staging model, psychotic disorders most often present first during the ultra high-risk stage (UHR) as attenuated symptoms, such as vague perceptual abnormalities. These sub-threshold symptoms later evolve into full-blown symptoms, including hallucinations, during the first-episode of psychosis (FEP) stage. After a period of psychotic symptom remission, a significant proportion of patients will experience a psychotic relapse, characterizing the multiple-episode of psychosis (MEP) stage. Studies comparing the different stages of the psychosis continuum are scarce and filling such a gap in the literature could help developing stage-adapted interventions. Objectives: This thesis globally set out to explore the heterogeneous and changing nature of psychosis. To do so, patients in the different stages of the psychosis continuum were compared on the aforementioned key clinical dimensions. Specific research questions that had a high translational potential for care were elaborated for each of the three studies comprised in this thesis. Study 1 aimed to estimate the prevalence of negative symptoms across the psychosis continuum. The objective of study 2 was to compare cognitive clusters across FEP and MEP patients to determine whether cognitive heterogeneity was comparable across stages. Finally, the goal of study 3 was to verify whether stage of illness moderates the association between cognitive capacity and insight into symptoms. Methods: In the first study, we reviewed the literature to compare the prevalence of each negative symptom across illness stages. For the second and third studies, we analyzed the data of FEP and MEP patients. In study 2, we used cluster analysis, while in study 3, we used structural equation modeling and moderated multiple regression analysis. Interpreted results: Forty-seven articles were included in study 1 and our analyses showed that the prevalence of negative symptoms decreased between the UHR and FEP stages to subsequently increase in the later stage. This indicates that negative symptoms are already present in the early stages and that they are most prevalent in the MEP stage. Results of study 2 revealed that cognitive heterogeneity was similar in FEP and MEP patients, with some patients presenting normal cognitive capacity, others displaying severe deficits generalized across all domains, and some exhibiting difficulties limited to specific domains (e.g., verbal memory). Larger proportions of FEP patients had normal cognitive capacity and more MEP patients presented with the generally impaired cognitive profile. This suggests that psychotic episode recurrence may have a detrimental impact on cognitive capacity. In study 3, results suggest that cognitive capacity explains a small proportion of symptoms unawareness' variance in both FEP and MEP patients, and that stage of illness does not moderate this association. Conclusions: Our results established that the clinical staging model can be useful in delineating the heterogeneous and changing nature of psychosis. Further, we propose that the current findings can help elaborate stage-specific interventions to minimize the cumulative detrimental effect of illness chronicity. " --
