Inhaltsangabe:Einleitung: Das Gesundheitswesen in Deutschland unterliegt derzeit einem grundlegenden Wandel. Ausgelöst und aufrechterhalten wird dieser Prozess durch die Mittelknappheit im Gesetzlichen Krankenversicherungssystem, unter anderem aufgrund der Veränderungen der Demographie und der Innovation in der Medizin. Unter der so entstandenen Knappheit wächst der Druck zur Ausschöpfung von Rationalisierungseffekten auf Seiten der Leistungserbringer. Speziell für die Krankenhäuser ergibt sich die Notwendigkeit, über Qualität, Leistung und Preis Wettbewerbsfähigkeit zu erlangen, um dem steigenden Konkurrenzdruck standhalten zu können. Durch die Einführung des neuen leistungsorientierten und pauschalierenden Vergütungssystems auf Grundlage der Diagnosis Related Groups (DRG), sehen sich die Krankenhäuser großen Herausforderungen gegenüber. Das DRG-System wird einen tief greifenden Veränderungsprozess auslösen. Nahezu alle Krankenhäuser werden ihre bisherigen Strukturen und Behandlungsabläufe überprüfen und gegebenenfalls neu strukturieren. Das Arbeiten im Krankenhaus wird sich inhaltlich, organisatorisch und auch vom Selbstverständnis her verändern, wenn die vormals karitativen Einrichtungen zunehmend nach unternehmerischen Gesichtspunkten geführt werden sollen. Nach Abschluss der Konvergenzphase zur Einführung der pauschalierten Vergütung im Jahr 2007 muss ein Krankenhaus seine historisch gewachsenen internen Strukturen soweit angepasst haben, dass es mit den dann geltenden einheitlichen Vergütungen kostendeckend wirtschaften kann. Die klinikinternen Abläufe müssen so gestaltet werden, dass sie einen kurzen, reibungslosen Aufenthalt der Patienten, aber auch eine Hinwendung zum Patienten und die Befriedigung seiner sozialen und kommunikativen Bedürfnisse ermöglichen können. Expliziter Grundsatz der Arbeit im Krankenhaus muss sein, alles auf die Interessen des Patienten auszurichten, ihn kostengünstig und gleichwohl medizinisch optimal zu versorgen sowie eine hohe Kundenzufriedenheit zu erreichen. Die Verwirklichung dieser Ziele setzt qualifiziertes Personal, effiziente Behandlungsabläufe und einen rationalen Mitteleinsatz voraus. Nur so kann eine hohe Produktivität erreicht werden. Es wäre falsch passende Lösungsansätze nur auf der Erlösseite, also aufseiten der zu erwarteten Zahlungen durch die Kostenträger zu suchen.
