Spinal instability is the inability of the various spinal structures to limit displacement under physiological loads in order to prevent neurological damage, incapacitating pain, and major deformity. There is no quantitative gold-standard definition for non-traumatic instability. The study objectives were to quantify unstable motion for the pediatric to elderly spine and to assess instability for ligament injury and multiple laminectomy. The novel instability metric, a collection of instability data, was produced from normal range of motion data and a published guideline for adult spinal instability. A high-fidelity finite element model of the adult lumbar spine was modified to test the sensitivity of singular ligament removal and size and location of multiple laminectomy to induce instability under physiological loads. Range of motion indicating instability increases in pediatric patients until the onset of puberty and then declines with age. Singular ligament removal and one-, two- and three-level laminectomy did not induce instability except in the case of facet capsular ligament removal. It is difficult and expensive to study spinal disorders in vivo making it advantageous to study them ex vivo. The spinal instability metric (SIM) will provide a standard for application with any spine model, clinical or experimental, to assess the risk of developing the instability-induced effects of pain, neurological damage and deformity.

Low back pain (LBP) affects many individuals worldwide. The established association between LBP and spine motor control has led to the development of many control assessment techniques. To understand the association between motor control and LBP, it is essential to understand the relationship between separate assessment techniques. Systems identification (SI) and local dynamic stability (LDS) are two methods commonly used to quantify spine control. SI provides a detailed description of control but uses linear assumptions, whereas LDS provides a "black box" non-linear assessment and can be quantified during dynamic movements. Although both SI and LDS techniques aim to measure the control of the spine, each employs different experimental setups and data processing strategies. Therefore, the purpose of this thesis was to compare the motor behaviour outcomes of SI and LDS quantification techniques. To do this, 15 participants completed two tasks (SI and LDS) in a random order. For the SI task, participants were seated and ventrally perturbed at the level of the 10th thoracic vertebrae (T10). They completed this task under instructions to resist the perturbations (resist condition) or relax and remain upright (relax condition). Admittance was represented using frequency response functions, and a validated neuromuscular control model quantified lumbar stiffness, damping and muscle spindle feedback gains. The LDS task involved participants completing three repetitive movement blocks consisting of flexion/extension, axial rotation, and complex movements. In each block, the maximum finite-time Lyapunov exponent (?max) was estimated. A stepwise linear regression determined that ?max during the rotation task was best predicted by SI outcomes in the relax condition (adjusted R square = 0.65). Many conditions demonstrated no significant relationship between ?max and SI outcomes. These findings outline the importance of a consistent framework for the assessment of spine control. This could improve clinical assessment efficiency as well as the understanding of the association between LBP and motor control.

Biomechanics of the Spine encompasses the basics of spine biomechanics, spinal tissues, spinal disorders and treatment methods. Organized into four parts, the first chapters explore the functional anatomy of the spine, with special emphasis on aspects which are biomechanically relevant and quite often neglected in clinical literature. The second part describes the mechanics of the individual spinal tissues, along with commonly used testing set-ups and the constitutive models used to represent them in mathematical studies. The third part covers in detail the current methods which are used in spine research: experimental testing, numerical simulation and in vivo studies (imaging and motion analysis). The last part covers the biomechanical aspects of spinal pathologies and their surgical treatment. This valuable reference is ideal for bioengineers who are involved in spine biomechanics, and spinal surgeons who are looking to broaden their biomechanical knowledge base. The contributors to this book are from the leading institutions in the world that are researching spine biomechanics. Includes broad coverage of spine disorders and surgery with a biomechanical focus Summarizes state-of-the-art and cutting-edge research in the field of spine biomechanics Discusses a variety of methods, including In vivo and In vitro testing, and finite element and musculoskeletal modeling
