Excerpt from Radium, Vol. 4: October, 1914 Only a few years ago there were but comparatively few men in America using the radium rays as a therapeutic agent. The greater number of physicians who even knew of radium regarded it as a mysterious substance notable mainly by reason of its exceeding rarity and the great difficulty with which it was to be obtained from the ore. Europe was the only producer and because of the demands over there, very little radium found its way to America. Meanwhile a great deal of pioneer work in the therapeutic applications of radium and other radioactive substances was being done and the interest in these substances and their effects was continually increasing. It was in 1913 at Halle that Bumm, Doederlein and Kroenig and Gauss announced the remarkable results which they had obtained in the treatment of uterine cancer by the application of massive doses of gamma rays from mesothorium and radium. These results were commented on by the whole medical world and the demand for the radioactive substances became so great that the supply for many months to come was soon exhausted and every effort was made to increase the radium output. No doubt the reports given at Halle were too enthusiastic and subsequently work has shown that the results obtained have not all been as successful as would have been expected. Those who had never used radium and had no conception of its power were the most bitter critics but in spite of their words it has come to be quite generally admitted that radium is of the greatest value in the treatment of many malignant and nonmalignant conditions. Excerpt from Radium, Vol. 4: February, 1915 Radium is likely to play its role so much the better, the more the neoplasm has been reduced to an appreciable minimum by a surgical operation. We may say also that the latter may be facilitated or even rendered possible by radium, which, in cer tain cases, modifies an inoperable neoplasm to such an extent that it becomes operable. Such cases, above all others, would be amenable to post-operative irradiation. In spite of our long period of personal practice we have been unable to formulate any statistics, owing to the fact that the cases differ so much from one another, and we can here' give only a general summary of facts reflecting the general impression which has been made upon us by the numerous cases of cancer of the uterus or of recurrence which we have treated by radium. Excluding cases in which the lesions have deeply invaded the true pelvis and the ganglia, and those in which extension to the bladder and rectum render any attempt at complete treatment of the neoplasm impossible, it may be said that the distressing symptoms and haemorrhage have invariably been greatly relieved by the application of radium. Although we emphasize the value of radium therapy, we do not for a moment intend to claim that radium plays the role in the cure of cancer. Our present object is to render obvious its value as a new therapeutic agent, which is capable of rendering services to the patient, such as can be rendered by no other method of treatment, excepting always those which can be ren dered by surgery in operable cases. Without wishing to revert here to the special practical facili ties oi the armamentarium of radium therapy, we think it, desir able, whilst doing justice to the painlessness of the applications, to point out a few manifestations inherent in the treatment, which it is essential to recognize and take into consideration. Even during the applications the patients may experience a sen sation of nausea, which in certain cases results in vomiting, but which otherwise disappears very rapidly. Excerpt from Radium, Vol. 4: A Monthly Journal Devoted to the Chemistry, Physics, and Therapeutics of Radium and Radio-Active Substances; November, 1914 With regard to the use of radioactive substances in the treat ment of cancer, Dr. Levin says: The-rationale of the radiotherapy in malignant tumors must be looked for in the so-called selective action of the rays on the cancer cell. The Roentgen rays and the rays of the radioactive substances are absorbed by the cells of the radiated tissue. Through the biochemical action of the'rays the cells become dis eased. The injury caused in the cells by the radiations may differ in degree in accordance with the quantity of the rays absorbed. Cells only slightly injured may recover while a severer injury may completely destroy the cell. Every cell of the animal or plant organism is susceptible to injurious action of the radiations. But the susceptibility varies in degree with the cells of different tissues. The cells of malignant tumors are a great deal more susceptible to the action of the rays than the cells of the normal tissues surrounding the tumor. If the rays are employed of a cor rect quality and in a sufficient quantity, it is possible to destroy all the cancer cells in a certain locality and at the same time leave the normal tissues either intact or so slightly injured, that they ultimately recover. This quantitatively selective influence of the rays may reach to a depth of at least three and one-half cm. Technique of Radium Therapy. The alpha and beta rays which constitute the greater part of the radiation are called soft rays and have little power of penetration. They are ah sorbed completely by the superficial layers of tissue, and the quan tity absorbed by each cell is so great that all the tissues are de stroyed, and the action of the rays is caustic. The hard gamma rays represent only about 1 per cent. Of the radiation, they pene trate into the deeper layers of tissue and exert the selective action on the cancer cells. It is possible that a small percentage of the hardest beta rays have a similar action. With the small quanti ties of radium used formerly the gamma rays had no influence on the tissues. The modern method of radium therapy consists in employing larger quantities of radioactive substances and apply ing them for a longer period of time. The soft caustic alpha and beta rays, which would be very injurious with the larger quanti ties of radium used, are filtered ofi by layers of some heavy metal which absorbs the soft rays. The methods vary with the different investigators some of them employing extremely large quantities of the substance as high as 800 mgm. Of mesothorium at one time. Recently it was shown that with such extremely large quantities the selective action may again be lost and the whole organism of the patient injured. It would lead too far to describe all the vari ous methods employed and the writer will present only his own method of application of radioactive substances. On the other hand, when the patient can be kept under observation until a cure is completed and the growth is proceeding slowly, there seems to be no reason why radium should not be used. In cases of this kind, however, it has the disadvantage of slowness, and there is also the difficulty, in most places, of obtaining the radium. But many patients prefer any amount of treatment to the use of the knife, a psychological factor that is heavily featured in the advertisements of the clever charlatans.
