Preoperative Neoadjuvant Chemotherapy written by Joseph Ragaz and has been published by Springer Science & Business Media this book supported file pdf, txt, epub, kindle and other format this book has been release on 2012-12-06 with Medical categories.

Despite recent advances in adjuvant therapies of cancer, the regi mens of postoperative adjuvant chemotherapy treatment which are presently available fail to cure the majority of cancer patients. Pre operative (neoadjuvant) chemotherapy represents a new approach in drug scheduling, based on sound theoretical, pharmacokinetic, and experimental principles. The preoperative timing of chemotherapy before definitive sur gery is not a minor change in the therapy of cancer. To be successful, large numbers of practitioners and their patients must participate. Substantial alterations of many aspects of the present management of cancer will have to follow. Therefore, before such therapy can be fully and routinely implemented, results of the novel treatment and its rationale have to be carefully evaluated. In preoperative treatment, other features will likely gain impor tance. For the first time, clinicians have a chance to follow the in vivo response of the tumor exposed to preoperative chemotherapy. The subsequent histological assessment of the tumor sample may likely become an important prognostic guide, permitting more re fined individual approaches to the planning of postoperative adju vant treatment. The value of such a treatment strategy can already be appreciated in the clinical setting, as seen from the therapy of osteosarcoma. Furthermore, preoperative chemotherapy might render previously inoperable tumors operable and hence resectable with a curative intention. The preoperative reduction of tumor bulk may also effectively decrease the need for more radical operations, permitting a more uniform adoption of conservative surgery.

While many comprehensive texts have been written on the treatment of breast cancer, the most common cancer among women, there are relatively few which cover in depth the prevention and early detection of the disease. The goal of this work is to present what experts in the ?eld feel is the current knowledge and future direction of breast cancer prevention and early detection. We begin Part I of the book with a review of risk factors, both genetic and environmental. We next review progress in the use of chemoprevention. Notably, chemoprevention risk reduction studies have led to FDA approval of two medications which measurably reduce disease incidence among women at increased risk, although with some risk of treatment related side effects. Newer agents in the pipeline, which may also reduce risk among normal risk women, are also discussed. Surgical risk reducing strategies complete the section on prevention, including both the bene?ts and downsides to this more aggressive approach. Even with aggressive prevention strategies, some women will develop breast cancer. For these women, early detection is critical to minimize disease spread and maximize long term survival. Part II of this book reviews current and upcoming approaches to early detection. Imaging strategies, including mammography, breast ultrasound, MRI, and PET imaging are reviewed. The potential for molecular tumor targeting to detect disease prior to the formation of a mass visible by anatomic imaging is presented.

This book is designed to capture and clinically review the comprehensive database of clinical research articles that support and do not support the utilization of a variety of dietary supplements and other complementary medicines that physicians are exposed to in their daily practice. The growing list of CAM products that could interfere with surgery (anesthesia, bleeding, outcomes...) and/or conventional medicines is very large and is provided in each section of the book. Additionally, the list of dietary supplements that could be utilized to improve quality of life for breast cancer patients is also emphasized. The various sub-specialty groups in breast are adequately represented, which allows for a physician to rapidly and thoroughly investigate their topic of interest regardless of whether the topic is prevention, treatment, or a specific side effect of treatment. The practical nature of Integrative Medicine for Breast Cancer: An Evidence-Based Assessment cannot be overstated. Chapters include a general overview of the CAM agent, whether or not it has data in medicine and oncology, and a list of potential drug interactions and specific clinical scenarios where it can be utilized or discouraged in the specialty. Thus, this book will become the gold standard evidence based text for use in teaching, not only for the students interested in oncology and breast cancer, but for all current oncology health providers.

Advances in anti-cancer chemotherapy over recent years have led to improved efficacy in curing or controlling many cancers. Some chemotherapy-related side-effects are well recognized and include: nausea, vomiting, bone marrow suppression, peripheral neuropathy, cardiac and skeletal muscle dysfunction and renal impairment. However, it is becoming clearer that some chemotherapy-related adverse effects may persist even in long term cancer survivors. Problems such as cognitive, cardiovascular and gastrointestinal dysfunction, and neuropathy may lead to substantial long term morbidity. Despite improvements in treatments to counteract acute chemotherapy-induced adverse effects, they are often incompletely effective. Furthermore, counter-measures for some acute side-effects and many potential longer term sequelae of anti-cancer chemotherapy have not been developed. Thus, new insights into prevalence and mechanisms of cancer chemotherapy-related side effects are needed and new approaches to improving tolerance and reduce sequelae of cancer chemotherapy are urgently needed. The present Research Topic focuses on adverse effects and sequelae of chemotherapy and strategies to counteract them.

This book provides a clinically useful resource for evaluation and management of the symptoms and issues that burden survivors of breast cancer. Improvements to breast cancer screening and treatment have resulted in more patients than ever before having been cured after local definitive and systemic therapies. Primary care providers and specialists must be increasingly familiar with the issues that breast cancer survivors routinely face. This is the first book to provide a single resource for common issues faced by breast cancer survivors from a truly multidisciplinary perspective; each chapter of this text is coauthored by at least one oncologist and one specialist outside the field of oncology in order to include the perspectives of relevant disciplines. User-friendly and clinically applicable to all specialties, individual chapters also include tables and figures that describe how best to conduct initial evaluation of the given symptom as well as an algorithm, where applicable, outlining the optimal management approach. Common Issues in Breast Cancer Survivors: A Practical Guide to Evaluation and Management empowers non-cancer specialists and practitioners who care for breast cancer survivors to address common issues that impact patient quality of life.
