The Advanced Study Institute on Regression of Atherosclerotic Lesions, sponsored by the North Atlantic Treaty Organization, was held in Maratea, Italy, September 11-13, 1983. The institute was designed to foster discussion of experimental data on the regression of atherosclerosis in animals and evidence of a similar phenomenon in humans. The participating scientists showed with anatomical and biochemical data, that atherosclerosis regresses in cholesterol fed animals after withdrawal of atherogenic stimuli. The existence of regression in humans was inferred from sequential angiographic visualization of arteries, but anatomical and biochemical characteristics of the process have not been defined. Two mechanisms are probably involved in such an angiographic regression. The first, which may occur when hyperlipidemia is partly or fully corrected or during undernutrition or other metabolic abnormalities, may be related to atherosclerosis changes noted in animals undergoing regression. The second type seems independent of changes in plasma lipid levels and may involve resolution of thrombi or other underlying arterial wall factors. The relative frequencies of the two mechanisms are unknown; whether they can occur simultaneously also warrants consideration. It is likely that an exciting milestone will be forthcoming when regression of atherosclerosis is unequivocally demonstrated in humans. Publication of these institute proceedings constitutes a valuable background for future developments.

Interest in the field of atherosclerosis research has broadened in recent years. However the main focus remains on the physiopathology of the arterial wall and on its interaction with blood constituents. The purpose of this NATO Advanced Study Institute on "Factors in Formation and Regression of the Atherosclerotic Plaque" was to discuss the following points: a) The physiopathology of the arterial wall; b) Animal models; c) Methods of studying the progression and regression of atherosclerotic lesions quantitatively; d) The role of lipoproteins, platelets, smoke, alcohol, etc. in the formation of atherosclerotic lesions; e) The pharmacological and dietary control of "risk factors." This volume is a collection of the most relevant presentations on these topics. We hope it will provide a background for young scientists as well as a stimulus for further research to biologists and clinicians. We wish to acknowledge the support of NATO and the Nutrition Foundation of Italy in organizing this Advanced Study Institute.

"Background Differences in shear stress form the basis of the differences in atherosclerosis predilection. Plaques form in regions of low, oscillating flow, whereas vessels exposed to high, steady shear stress tend to remain lesion-free due to the release of protective factors from the endothelial layer, primarily nitric oxide. While progressing plaques are characterized by the persistent entry of immune cells in the vascular wall, regressing plaques are characterized by 2 key features, effective emigration and efferocytosis of these immune cells. We hypothesize that shear stress reverses atherosclerosis by promoting macrophage exit from plaque (emigration) and more efficient uptake of cellular debris (efferocytosis). Methods In vivo: The arteriovenous fistula (AVF) is a surgical model established in the laboratory, whereby the right carotid artery is anastomosed into the right jugular vein, creating an arterio-venous shunt. We studied atherosclerotic plaques in the brachiocephalic artery (BCA), the plaque-prone vessel that links the aorta with the right carotid artery. Low density lipoprotein receptor (LDLR) knockout (KO) mice were placed on a high-fat diet. Sham and AVF surgery was performed at week 12 and mice were kept on a high-fat diet for a further 4 weeks (week 16). Control mice were sacrificed at week 12. In vitro: Endothelial cells (ECs) were co-cultured with macrophages in a shear stress cone system wherein ECs were exposed to high or low shear stress and macrophages were exposed to the EC effluent. Results The AVF procedure increases the shear stress in the BCA but does not alter serum lipid levels. Using this AVF model in mice, we found that elevated shear stress can reverse the atherosclerotic process, reducing plaque size. This increased shear stress also leads to decreased necrotic core size, increased matrix metalloproteinase (MMP) activity, increased MMP-9 expression and decreased collagen IV in addition to increased macrophage migration. MMP or endothelial nitric oxide synthase inhibition abolished shear stress-induced plaque regression. MMP inhibition also decreases macrophage migration with specific MMP-9 inhibition having the same effect. Using the in vitro shear stress co-culture apparatus to mimic the plaque environment, we found that uptake of apoptotic cells by macrophages was higher in the high shear conditions with MMP inhibition having no effect. However, a cytokine array on the co-culture effluent revealed a potential role for specific cytokines. Conclusions Our findings suggest that not only is shear stress protective against plaque development but it can effectively reverse the atherosclerotic process without interfering with plasma lipid levels. Shear stress acting on ECs may influence the cells within the plaque by increasing macrophage mobility and efferocytosis, the combination of which likely leads to plaque regression. These results and future work will help us further identify the molecular mechanisms that cause the atheroprotective effects of shear stress, in hopes of identifying new therapeutic targets for the treatment of atherosclerosis and cardiovascular diseases. " --

Even though numerous questions with regard to the pathogenesis of athero sclerosis have not yet been answered, the accumulated evidence indicates significant regression of lesions in experimental animals. This is discussed extensively in this monograph, as are the mechanisms involved in regression of lesions. Whether human atherosclerosis has the potential for regression appears to be the most important, but at the same time the most difficult question to answer. Contrary to experimental atherosclerosis in animals, which can be produced and which can regress within a few months, human lesions in general develop slowly over many years. Therefore, measures aimed at modifying this process may also require many years to be successful. In addition, repeated direct examination of lesions in the human is usually not possible. Nevertheless, recent reports in patients with hyperlipoproteinemias indicate that pronounced and maintained control of hyperlipidemias may lead, even within months, to regression as evidenced by angiography or sophisticated measurements of peripheral circulation. The monograph is divided into two sections. The first will deal with of lipid deposition in the arterial wall, whether "atherogenesis": mechanisms or not there is evidence of monoclonal origin of human atherosclerosis plaques, cell culture and factors that stimulate smooth muscle proliferation, and animal models of atherogenesis. This section is concluded with a discussion of dietary factors other than lipids in atherogenesis.
