Despite the proliferation of pain clinics and various pain-oriented therapies, there is an absence of data supporting any substantial change in the statistics regarding the incidence, development and persistence of pain. As renowned pain clinician and scientist Daniel M. Doleys argues, there may be a need for a fundamental shift in the way we view pain. In this thoughtful work, Doleys presents the evolving concept and complex nature of pain with the intention of promoting a broadening of the existing paradigm within which pain is viewed and understood. Combining neuroscience, psychology, and philosophy of science, this book reviews the history of pain and outlines the current concepts and theories regarding the mechanisms involved in the experience of pain. Experimental and clinical research in a broad array of areas including neonatal pain, empathy and pain, psychogenic pain, and genetics and pain is summarized. The notion of pain as a disease process rather than a symptom is highlighted. Although there is a continued interest in activation of the peripheral nociceptive system as a determining factor in the experience of pain, the growing appreciation for the brain as the intimate 'pain generator' is emphasized. The definition of consciousness and conscious awareness and a theory as to how it relates to nociceptive processing is discussed. Finally, the author describes the potential benefit of incorporating some of the concepts from systems and quantum theory into our thinking about pain. The area of pain research and treatment seems on the precipice of change. This work intends to provide a glimpse of what these changes might be in the context of where pain research and therapy has come from, where it currently is, and where it might be headed.

Background and aims. This systematic review examined the extent to which Religiosity/Spirituality (R/S): (1) are associated with pain, function, pain-related beliefs (beliefs), coping responses and catastrophizing in people with chronic pain; and (2) moderate the association between beliefs, coping and catastrophizing, and pain and function. Methods. Experimental and observational studies examining at least of the above mentioned research questions in adults with chronic pain were eligible. Two reviewers independently performed eligibility screening, data extraction, and quality assessment. Results. Twenty-four studies were included. Most studies focused on the association between religiosity/spirituality and pain or function. When significant associations between religiosity/spirituality and psychological function were positive; and between religious/spiritual well-being and pain and physical function were negative, but weak. Few studies compared religious groups regarding criterion variables, and examined the associations between religiosity/spirituality and beliefs/coping/catastrophizing; none examined the moderation role of R/S. Conclusions. Religiosity/spirituality are associated with pain and psychological function in people with chronic pain. Viewing oneself as being u201cspiritual,u201d regardless of religiosity and religious affiliation, are a useful resource for psychological adjustment. This suggests the need for considering the role of religiosity/spirituality on psychological adjustment in context of patient care. An improved understanding of the role that religion plays in the lives of individuals with chronic pain would be facilitated by (1) the development of a theoretical model informed by the findings from this review and (2) efforts to clarify and standardize definitions and measure of the key domains, ideally based on a theoretical model. One of the most debilitating conditions that currently exists is chronic pain. With the multitude of pain-related consequences that can be experienced, the biopsychosocial perspective examines the chronic pain experience using a multifactorial approach that examines all aspects of life impacted by the chronic pain experience (Gatchel et al., 2007). In examining aspects that have impacted the chronic pain experience, religion has been found to be a factor. Previous research has shown that religious practitioners experience a high likelihood of turning towards religion in an effort to seek an understanding regarding what is happening to them (Pargament & Raiya, 2007). With music being a common pain intervention, being able to choose what music to listen to has been found to be related to positive outcomes (Mitchell et al., 2007). For religious individuals, religious music has been found to be a coping mechanism in response to stressful situations (Safara & Samanesadatsadidpoor, 2014). The exact mechanism for the relationship is unknown, however, when both music and religion are important, the effect of music as a coping mechanism may be amplified (Hilliard, 2005). The main goal of this study is to examine how religious and non-religious music impacts the chronic pain experience. In addition, religion and the chronic pain experience will be examined. Participants were recruited through Amazon's Mechanical Turk (MTurk) in order to gather an adult sample of individuals with pain. Statistical analyses found that individuals who scored high on measures of religiousness also reported listening to music that reminded them of their religion. It was also found that there is a strong cognitive associative context in which music is used for pain management. Implications from this research include providing resources for religious individuals experiencing chronic pain, best practices for studying religion in a psychological context, and tailoring non-pharmacological pain interventions.
