Bleeding from the liver surface during hepatic transection is one of the main factors affecting mortality and morbidity of liver resection. For this reason, numerous devices have been developed that employ a variety of techniques to minimize parenchymal damage and so improve the safety of resection. This book describes all the devices that are currently available for hepatic transection via open, laparoscopic, and robotic approaches. Procedures are explained and illustrated step by step using informative color figures and photographs. This landmark reference in the field will be an ideal guide for liver surgeons as well as a valuable tool for students, residents, and general surgeons.

Minimally invasive surgery has experienced a significant expansion in the last decades. Robotic surgery has evolved in parallel to traditional laparoscopic surgery offering additional technical advantages. Some specific aspect of Hepatobiliary Surgery led to a limited implementation of minimally invasive liver surgery in the early years of laparoscopic surgery whilst we are experiencing an exponential increase in the use of minimally invasive approaches to this type of intervention. In this chapter we describe the key aspect of robotic liver surgery with a meticulous description of the supporting evidence, its limitation and future perspectives.

This book intends to provide a comprehensive and up-to-date description of the newest techniques in minimally invasive liver surgery. The treatment of liver diseases comprises a significant component of the practice of any general surgeon working either in the academic, general and community hospitals. With the rapid expansion of knowledge and technology also the liver has been approached laparoscopically in a few specialized centers with well recognized advantages for the patients. Now this large amount of experience will be made universally available through this book. After a concise description of the main tools and technology necessary to carry out a safe laparoscopic and/or robot-assisted liver surgery, details of operative techniques will be illustrated. Each chapter will consist of an introduction, a summary of the indications and contraindications of specific operative procedures and a detailed description of each surgical procedure, including the potential complications of both the surgery and the postoperative care. A special effort will be done to provide informative and accurate illustrations and pictures, that will be completed by the DVD containing the movies illustrating the technical procedures performed by the leading European liver surgeons.

Robotic liver surgery is an evolving specialty within liver surgery. The robotic platform allows some of the limitations in both open and laparoscopic surgery to be overcome. Indeed as the technology develops there is scope for the number of robotic liver resections to increase as well as their complexity. In this chapter we discuss the current robotic platform, review the current role of robotics in liver surgery and review the available data in the literature on patient outcome.

"The current thesis aims to improve the imaging work-up of patients with colorectal liver metastases by defining the exact role of liver MRI within the imaging pathway for colorectal liver metastases. Additionally, this thesis presents results of the implementation of laparoscopic and robotic liver surgery in order to further guide the safe dissemination of both approaches on a national and international scale. It also aims to critically assess the outcomes of laparoscopic and robotic liver resections to improve patient outcomes after liver surgery. Part I focuses on the imaging work-up of patients with colorectal liver metastases amenable to local treatment. Part II describes and assesses the implementation of laparoscopic and robotic liver surgery. In part III, an attempt to improve outcomes of laparoscopic and robotic liver surgery is undertaken. In conclusion, significant progress has been made in the diagnostic work-up of patients with CRLM and the implementation and outcomes of laparoscopic and robotic liver surgery. Hopefully, this thesis stimulates surgeons, oncologists, abdominal and interventional radiologists worldwide to consider the routine use of liver MRI in the diagnostic work-up of patients with CRLM amenable to local treatment. Furthermore, this thesis serves as a stimulus for surgeons worldwide to further enhance their laparoscopic and robotic liver surgery practice, striving for optimal patient outcomes."--

Glissonean Pedicles Approach In Minimally Invasive Liver Surgery written by Benedetto Ielpo and has been published by Springer Nature this book supported file pdf, txt, epub, kindle and other format this book has been release on 2023-12-30 with Medical categories.

Aim of this book is to present the main minimally invasive approaches to the Glissonean pedicles. This kind of technique for hepatectomy has been gaining interest in recent years and may represent the ideal method to achieve anatomical liver resections. Currently the most performed procedure for liver surgery is still the classic intrafascial approach in which, after entering the Glissonean sheath, the portal triad elements are singularly dissected and isolated. However, this is a quite challenging technique to pursue in a minimally invasive setting. Nowadays the laparoscopic and robotic approach for liver resections are increasing worldwide and the intra or extra hepatic Glissonean approach, in which the portal triad elements are dissected and isolated “en bloc” without entering the Glissonean sheath, can facilitate the surgical technique. Furthermore this strategy allows better identification of the ischemic area and could decrease blood loss during dissection, as the liver inflow is kept under control. This book includes a step-by-step description of each pedicle isolation and section, with original laparoscopic and robotic pictures and videos. HPB surgeons who are interested in this technique may find these pages, written in cooperation with some of the most prominent worldwide experts, to be highly informative and valuable.

Liver Transplantation And Hepatobiliary Surgery written by Umberto Cillo and has been published by Springer Nature this book supported file pdf, txt, epub, kindle and other format this book has been release on 2019-08-20 with Medical categories.

In the past, liver resections and liver transplantations were performed by different surgical teams, with very little interplay between the two. However, the evolution toward more complex operations called for an increasingly intense interaction between these surgical techniques: split liver and living donor liver transplantation have become popular in the transplant community, utilizing the Coinaud segmental anatomy in a very sophisticated way, while portal and arterial resection and reconstruction have become indispensable methods for treating Klatskin tumors infiltrating the hepatic hilum. This book offers a complete overview of the connections between liver transplantation and hepatobiliary surgery. It focuses on ex situ techniques for resection of tumors at the suprahepatic confluence, with or without venous reconstruction, and on total vascular exclusion of the liver with the aid of a venovenous bypass that should be a surgical option for referral centers in hepatopancreatobiliary surgery. By taking into consideration the development of minimally invasive liver surgery, it will introduce readers to a variety of new perspectives such as: vascular exclusion techniques with or without extracorporeal circulation, hemodynamic implications and lessons learned from liver transplant surgery, and technical details on pediatric transplantation and its informative role in modern biliary tract surgery.
