Why it is very important to read this book? Answer is, it is very important to read this book because Heart health is very important to us. Heart attack or sudden death from the heart attack has become so rampant in India. It is affecting more and more youngish male population. Women have some protection from the heart attack until they are in their reproductive age, but they still have some amount of risk. Our recommendation is to start screening all the males at age 30. This will give us a baseline. And continue every five years with three most important, simple, noninvasive, not at all costly, Tests. If all these three tests are normal, then I can give you in writing that you will not have heart attack for the next five years. Statistically, it is not possible. Why these three tests are important? These tests are important because all these three different tests give us complementary information. As far as ECG/EKG is concerned, it gives us all the information about the electrical conductivity of the heart and our ECG is like our fingerprint. I can guarantee that if you take your ECG every year and put one above the other, they will be exactly the same. ECG helps us to understand the function of the two electrical nodes in our heart, which control the rhythm and the rate of the rhythmic contraction and relaxation of the heart. We all know that, in a heart attack, we immediately do ECG and if you are having a heart attack, you can see changes in the ECG, we can pretty much figure it out. But why to wait for that point when we are really having heart attack, because we may not have more than 5 to 10 minutes to recover from the heart attack and if our brain does not get blood and oxygen for several minutes, our brain is never going to come back, even though if our heart comes back, and we may not be alive anymore. Since we will never have 5 minutes, so our hope is that by doing three simple tests, we can really, statistically, almost 100% anticipate what is going to happen over next five years and if we see any negative information, we can always fix it in time, seeking the help from the best heart doctors, but we need information and time.

Different artificial tools, such as heart-pacing devices, wearable and implantable monitors, engineered heart valves and stents, and many other cardiac devices, are in use in medical practice. Recent developments in the methods of cardiac pacing along with appropriate selection of equipment are the purpose of this book. Implantable heart rate management devices and wearable cardiac monitors are discussed. Indications for using specific types of cardiac pacemakers, cardiac resynchronization therapy devices, and implantable cardioverter defibrillators (ICDs) are of interest and their contraindications are considered. Special attention is paid to using leadless devices. The subcutaneous ICD obviates the need for transvenous leads and leadless pacemakers are entirely implantable into the right ventricle. Finally, applications of user-friendly wearable devices for the detection of atrial arrhythmia are debated.

This book is a comprehensive overview of electrocardiography and the major effects of current cardiac pharmacological therapy on electrocardiography. The text is based on work presented at the International Symposium on Non-invasive Cardiovascular Diagnosis and Therapy, held in May, 1987 at the University of Cologne. The theme of the book is to review, in broad clinical perspective the current state-of-the-art of electrocardiography as it pertains to standard electrocar diograms, exercise testing, ambulatory electrocardiography, electrocardiographic telemetry, and high resolution electrocardiography. Furthermore, advance!, in cardiac drug therapy in relation to diuretics, beta blocking drugs, antiarrhythmic agents and thrombolytic agents are reviewed. The emphasis of the conference and this book is to review the clinical state-of-the-art information and applications in this regard. In the initial section on electrocardiography, Dr. Spodick reviews our present day physiologic and pathophysiologic understanding of systolic time intervals, and how they are affected by a variety of cardiac disease states and pharmacologic agents. Dr. Ellestad examines problems and provides pragmatic tips on exercise testing in the diagnosis of coronary artery disease, and advances in exercise scores and computer analysis. Dr. Graboys reviews the value of exercise testing in the diagnosis and management of patients with serious ventricular arrhythmias. Dr. Kellermann presents the complimentary role that exercise testing plays in com prehensive follow-up therapy of the cardiac patient, and the use of exercise for work and physical training. Detailed information concerning the interaction of cardiac rehabilitation and ventricular arrhythmias are examined.

The underlying technology and the range of test parameters available are evolving rapidly. The primary advantage of POCT is the convenience of performing the test close to the patient and the speed at which test results can be obtained, compared to sending a sample to a laboratory and waiting for results to be returned. Thus, a series of clinical applications are possible that can shorten the time for clinical decision-making about additional testing or therapy, as delays are no longer caused by preparation of clinical samples, transport, and central laboratory analysis. Tests in a POC format can now be found for many medical disciplines including endocrinology/diabetes, cardiology, nephrology, critical care, fertility, hematology/coagulation, infectious disease and microbiology, and general health screening. Point-of-care testing (POCT) enables health care personnel to perform clinical laboratory testing near the patient. The idea of conventional and POCT laboratory services presiding within a hospital seems contradictory; yet, they are, in fact, complementary: together POCT and central laboratory are important for the optimal functioning of diagnostic processes. They complement each other, provided that a dedicated POCT coordination integrates the quality assurance of POCT into the overall quality management system of the central laboratory. The motivation of the third edition of the POCT book from Luppa/Junker, which is now also available in English, is to explore and describe clinically relevant analytical techniques, organizational concepts for application and future perspectives of POCT. From descriptions of the opportunities that POCT can provide to the limitations that clinician’s must be cautioned about, this book provides an overview of the many aspects that challenge those who choose to implement POCT. Technologies, clinical applications, networking issues and quality regulations are described as well as a survey of future technologies that are on the future horizon. The editors have spent considerable efforts to update the book in general and to highlight the latest developments, e.g., novel POCT applications of nucleic acid testing for the rapid identification of infectious agents. Of particular note is also that a cross-country comparison of POCT quality rules is being described by a team of international experts in this field.

Recent years have seen a marked increase in cardiovascular computed tomography (CT) imaging, with the technique now integrated into many imaging guidelines, such as those published by ESC and NICE. Rapid clinical and technological progress has created a need for guidance on the practical aspects of CT image acquisition, analysis and interpretation. The Oxford Specialist Handbook of Cardiovascular CT, now revised for the second edition by practising international experts with many years of hands-on experience, is designed to fulfil this need. The Handbook is a practical guide on performing, analysing and interpreting cardiovascular CT scans, covering all aspects from patient safety to optimal image acquisition to differential diagnoses of tricky images. It takes an international approach to both accreditation and certification, highlighting British, European, and American examinations and courses. The format is designed to be accessible and is laid out in easy to navigate sections. It is meant as a quick-reference guide, to live near the CT scanner, workstation, or on the office shelf. The Handbook is aimed at all cardiovascular CT users (Cardiologists, Radiologists and Radiographers), particularly those new to cardiovascular CT, although even the advanced user should find useful tips and tricks within.
