Sometimes, a ‘winging scapula’ reveals a disabling scapulothoracic condition. Sometimes it does not. Often, despite no observable winging, debilitating scapulothoracic conditions go undiagnosed or misdiagnosed. This “shoulder” problem is poorly understood. Disruption of essential scapular connections within the chest wall results in multiple, seemingly unrelated symptoms, obfuscating the true source of the problem. Scapular dyskinesis can masquerade as a neck problem, a back problem, a chest, head or arm problem. Current musculoskeletal assessments do not identify an isolated scapulothoracic condition unless there is obvious deformity or X-ray evidence of a fracture. In 2012, Janet Delorme, a physiotherapist with over thirty years’ experience in orthopedic assessment and treatment, suffered a peculiar loss of movement and strength in her arm. Only a physiotherapist would puzzle over this problem. Her curiosity and persistence, her specialized knowledge and experience, andher unique opportunity to truly understand this problem from the “inside” has unlocked an entirely new understanding of this complex biomechanical region. This assessment method is presented in an easy-to-use format, and it can be added to an existing assessment protocol. It is designed for any clinician who has the desire to learn how to recognize and manage a condition that may otherwise go undiagnosed.

This book was written by a physiotherapist who became the patient. From her unique perspective, Janet Delorme recognized and understood a complex problem that might otherwise go undiagnosed. Do these symptoms sound familiar? * Cannot tolerate repetitive/forceful arm movement, especially overhead. * Arm feels “weak” * Fatigue, shortness of breath, sense of arm “disconnect” or a twisted body * Pain in shoulder but also in the back, neck, arm or head. These symptoms suggest instability of the chest wall, the most familiar cause of which is long thoracic nerve palsy. With an unstable chest wall, diagnostic tests of the “shoulder” are often negative and complaints of fatigue and shortness of breath are unsubstantiated from a medical perspective. This complex problem will often require patients and therapists to work together. This book was written for both the therapist and the patient. It provides insightful information about this condition and explains how to modify activities and lifestyle to manage symptoms and restore a “new normal”. The book also teaches key elements necessary for recovery: movement principles and exercise programs. Not all people recover. Many make a full recovery. Both outcomes are addressed with advice for every step of the recovery process.

This unique book - the first of its kind exclusive on disorders of the scapula - is a concise but comprehensive summary of the evidence that will enable clinicians to understand the scapula from its functions to its dysfunctions and includes clinical guidelines and pearls to improve the clinician’s competencies for the treatment of shoulder disorders. Organized logically, the book opens with a review of the baseline mechanics and pathomechanics of the scapula, proceeds to evaluation, then describes in detail the association of the scapula with specific shoulder problems, including rotator cuff disease, labral injuries, glenohumeral and multidirectional instability, clavicle fractures, acromioclavicular joint separation, and shoulder arthrosis. Subsequent chapters cover scapular muscle detachment, neurological injuries and winging, scapular fractures and snapping scapula, in addition to basic and complex rehabilitation strategies. Each chapter includes a summary section with clinical pearls. In the past, in-depth research and expertise regarding the scapula was minimal, but a widening interest has resulted in a volume of literature that makes it possible and imperative that it be collected in a single volume. Disorders of the Scapula and Their Role in Shoulder Injury will be an excellent resource for orthopedic and trauma surgeons, residents and fellows.
