In the autumn of 1957, Stanley R. Dean proposed the creation of a founda tion on schizophrenia to a group of friends. The purpose was to bring the illness of schizophrenia before the public's attention and out of the shameful anonymity in which it was buried. Stanley Dean recognized the major public health importance of this illness and was determined to have an impact on it during his lifetime. By December of that year, the decision was made to create an organization called Research in Schizophrenia Endowment (RISE). Within less than two months articles of incorporation for RISE were issued by the State of Connecticut. By the latter part of March 1958, Dean was successful in getting Congress to provide $1,300,000 for research into schizophrenia. This was the first time that Congress had targeted funds specifically for research on this disorder. Within a year, by April 1959, Dr. Dean was instrumental in getting Congress to increase its appro priation to the NIMH by more than $8,000,000 with the specific instruction that approximately one-half of the total research funds of the NIMH be used for projects relating to schizophrenia. In the spring of 1960, RISE relinquished its corporate charter and became an integral part of the Research Foundation of the National Association for Mental Health. This merger occurred before RISE had implemented its planned annual award program for outstanding research in schizophrenia. I am honored to introduce readers to this extraordinary volume, the first in the annual International Perspectives Series: Psychiatry, Psychology, and Neurosciences. This particular volume on the Recent Advances In Schizophrenia stems from the Third Annual Pennsylvania Conference on Schizophrenia, held in March, 1988. At that international meeting, some of the most esteemed researchers in the field surveyed our current under standing of schizophrenia. Collectively, their presentations capture the excitement of a research field launching into a stage of rapid worldwide advancement. The last decade has seen an enormous refocusing of scientific effort on schizophrenia, directed, in large measure, by the National Institute of Mental Health (NIMH). Our understanding of this mysterious, disabling disorder in all its diversity has been enhanced by a host of technologies, including new neuroimaging techniques, cognitive psychology, molecular genetics, and anatomical, biochemical, and molecular neuropathology. The resulting growth spurt in our knowledge constitutes the essence of the conference and of this volume. As the following pages amply docu ment, it has been a decade of great progress, one in which we can take great pride. The science of our field has matured to the point that a major research thrust is now feasible-one likely to have enormous benefits for schizophrenic patients and their families. The mental health movement of early release into the community of the chronic schizophrenic has been based on a set of old theoretical assumptions and expectations which, when tested on the community level, failed to meet the desired results. On the contrary, the first visible outcome of deinstitutionalization was the revolving door policy with the patients repeatedly in and out of the hospital, changing their previous status of inpatient to the new one of pseudoam bulatory. Yet, this would not be a serious problem if the life of the patient in the community, in between rehospitalizations, was beneficial to him and to the community. However, the quality of life experienced by the patients in the community appears to be deplorable. Available statistics indicate that over 70% of chronic schizophrenics discharged into the community live a marginal, unproductive, aimless life in dilapidated hotels or private proprietary homes. Certainly, though there are various scattered community programs for the rehabilitation of the schizophrenic, the results are far from encouraging. With these basic facts in mind, we have to ask ourselves: What went wrong with deinstitutionalization? Apparently in the process of hasty deinstitutionaliza tion, too many issues were overlooked by the community mental health planners. The most critical factor neglected by the policy makers was the establishment of more realistic criteria for the patient who can or cannot function in the community.
