Dr Watson is called upon late at night by a female friend of his wife. Her husband has been absent for several days and, as he is an opium addict, she is sure he has been indulging in a lengthy drug binge in a dangerous East End opium den. Frantic with worry, she seeks Dr. Watson's help in fetching him home. Watson does this, but he also finds his friend Sherlock Holmes in the den, disguised as an old man, trying to extract information about a new case from the addicts in the den. Mr. Neville St. Clair, a respectable and punctual country businessman, disappears. Making the matter more mysterious is that Mrs. St. Clair is sure that she saw her husband at a second-floor window of the opium den, in Upper Swandam Lane, a rough part of town near the docks. He withdrew into the window immediately, and Mrs. St. Clair is sure that there is something wrong. She tries to enter the building, but her way is blocked by the opium den's owner, a lascar. She fetches the police, but they cannot find Mr. St. Clair. The room, in the window of which she saw her husband, is that of a dirty, disfigured beggar known to the police as Hugh Boone. The police are about to put this report down as a mistake of some kind when Mrs. St. Clair spots and identifies a box of wooden bricks that her husband said he would buy for their son. A further search turns up some of her husband's clothes. Later, his coat, with the pockets full of several pounds' worth of pennies and halfpennies, is found in the Thames, just below the building. The beggar is arrested and locked up at the police station, and Holmes initially is quite convinced that Mr. St. Clair has been the unfortunate victim of murder. However, several days after Mr. St. Clair's disappearance, his wife receives a letter in his own writing. The arrival of this letter forces Holmes to reconsider his conclusions, leading him eventually to an extraordinary solution. Taking a bath sponge to the police station in a Gladstone bag, Holmes washes Boone's still-dirty face, causing his face to be revealed - the face of Neville St. Clair! Upon Mr. St. Clair's immediate confession, this solves the mystery, and creates a few problems.

Watson visits his friend Holmes at Christmas time and finds him contemplating a battered old hat, brought to him by the commissionaire Peterson after the hat and a Christmas goose had been dropped by a man in a scuffle with some street ruffians. Peterson takes the goose home to eat, but later returns to Holmes with a blue carbuncle his wife had found in the bird's crop (throat). Holmes makes some interesting deductions concerning the owner of the hat from simple observations of its condition, conclusions amply confirmed when an advertisement for the owner produces the man himself: Henry Baker. Holmes cannot resist such an intriguing mystery, and he and Watson set out across the city to determine exactly how the jewel, stolen from the Countess of Morcar during her stay at a hotel, wound up in a goose's crop. The man who dropped the goose, Mr. Henry Baker, comes to reclaim his hat in response to Holmes' advertisement. Holmes drops hints about how he saved the "innards" of the goose, but Baker fails to respond to them, simply saying that he is afraid goose remains are not much use. He does, however, give Holmes valuable information, eventually leading him to the conclusive stage of his investigation, at Covent Garden. Holmes offers a fresh goose to Henry Baker, who responds with gladness and departs, whereupon Holmes tells Watson that Baker is eliminated from the suspect list as he obviously knows nothing about the carbuncle. At Covent Garden, a salesman named Breckinridge gets angry with Holmes, complaining about all the people who have pestered him about geese sold recently to the landlord of the Alpha Inn. Clearly, someone else knows that the carbuncle was in a goose and is looking for the bird.

"To Sherlock Holmes she is always 'the woman.'" So begins "A Scandal in Bohemia," the first story in the collection. Irene Adler is "the" woman because she is the only person ever to have outwitted Holmes. The King of Bohemia fears that he will be blackmailed by Adler, his former lover, who has kept some compromising love letters and a photograph. However, she manages to turn the tables on the detective, retaining the photograph to ensure her own safety. Other highlights in the collection are the eerie "The Red-Headed League," where a red-headed man is offered employment (copying entries from Encyclopaedia Britannica) by the League as a ruse to keep him occupied while criminals dig a tunnel from the cellar of his premises to a bank. In "The Man with the Twisted Lip," Holmes's help is enlisted to solve the mystery of the disappearance of Mr. Neville St. Clair. His wife has seen him at the window of an opium den in a rougher part of town, but the police are unable to find anyone but a beggar. Yet the London that features in these stories manages to resist the idea that the city is sublime, that it is too large for any one person to be able to comprehend. Holmes and Watson represent Conan Doyle's bourgeois remedy to the terrifying and seemingly endless late 19th-century expansion of urban and industrial civilization.
