Maxillary sinus floor augmentation (also termed sinus lift, sinus graft, sinus augmentation or sinus procedure) is a surgical procedure which aims to increase the amount of bone in the posterior maxilla (upper jaw bone), in the area of the premolar and molar teeth, by lifting the lower Schneiderian membrane (sinus membrane) and placing a bone graft.When a tooth is lost the alveolar process begins to remodel. The vacant tooth socket collapses as it heals leaving an edentulous (toothless) area, termed a ridge. This collapse causes a loss in both height and width of the surrounding bone. In addition, when a maxillary molar or premolar is lost, the floor of the maxillary sinus expands, which further diminishes the thickness of the underlying bone. Overall, this leads to a loss in volume of bone that is available for implantation of dental implants, which rely on osseointegration (bone integration), to replace missing teeth. The goal of the sinus lift is to graft extra bone into the maxillary sinus, so more bone is available to support a dental implant.

Background: Bone grafting materials are widely applied for maxillary sinus augmentation nowadays. However, it is still controversy regarding what grafting material is the most suitable.Aim: To compare the potential of platelet-rich fibrin (PRF) and deproteinized bovine bone mineral (Bio-Oss) in augmenting bone volume after maxillary sinus lift with simultaneous implant placement.Material and Methods: 37 patients with a total of 40 sites of maxillary sinus lift surgery were included in this study. The 40 sites were randomly divided into two groups, the PRF group and the Bio-Oss group. All sites underwent transalveolar maxillary sinus lift using a CAS kit. PRF was placed into the elevated sinus cavity in the PRF group. while Bio-Oss was placed in the Bio-Oss group. Dental implants were installed simutaneously. Cone beam computed tomography (CBCT) scan was performed preoperatively, immediately after surgery and 6 months after surgery to measure Schneiderian membrane thickness (SMT) , residual bone height (RBH), sinus width (SW), implant protrusion (IP), the height of sinus lift and intra-sinus bone gain. The integrity of Schneiderian membrane was checked by CBCT.Results: In the control group, length and diameter of implants were 10u00b10mm and 4.57u00b10.31mm, respectively, and the corresponding values in thePRF group were 10u00b11.19 mm and 4.59u00b10.39 mm, respectively. In the control group, Schneiderian membrane thickness, sinus width and residual bone height were 1.02u00b11.01 mmuff0c13.40u00b12.38 mm and 5.83u00b11.69 mm, respectively, and the corresponding values in the PRF group were 2.52u00b13.49 mm, 13.51u00b12.99 mm and 6.69u00b11.42 mm, respectively. In the control group, implant protrusions, the height of the maxillary sinus lift and bone gain were 3.44u00b10.787mm, 7.56u00b13.37mm and 4.65u00b12.65mmuff0crespectively, and the corresponding values in the PRF group were 3.44u00b10.787mm,7.56u00b13.37mm and 4.65u00b12.65mmuff0crespectively. No significant difference was detected between the two groups for all the tested parameters. Statistical analysis showed that bone gain was related to the height of sinus elevation, but not strongly related to age, SMT, sinus width (SW), RBH and implant protrusion (IP).Conclusion and Clinical implications: Both materials were beneficial for new bone formation after maxillary sinus elevation and the higher the maxillary sinus lifted, the more bone volume was gained.

Reconstruction of posterior maxillary edentulous ridge is a common challenge with insufficient bone height. Lateral window approach for sinus augmentation technique is a well-documented. Various biomaterials grafted sinus have been used such as xenografts, allografts, or composited graft. These biomaterial has demonstrated good outcomes, but most the grafts only with osteoconductive properties and had prolonged graft-healing time, therefore, the amount of new mineralized bone could be compromised. The recently reported that the sinus augmentation without bone graft might be considered effective impredictable bone formation. The advantages of one stage implant placement without bone grafts in sinus lift procedure are easily to identify the new bone formation around the implant, avoid bone grafts related infection complication, and reduce costs effect. The used L-PRF in sinus lift procedure may enhancing osteoprogenitor cells and modulate hard tissue transformation. The aim of this study was a case series of sinus augmented with xenograft, graftless, CGF + xenograft, or CGF alone ,to compare the sinus membrane elevation around implant volume stability and implant survival rate over 5-year follow up outcomes. Totally 10 patients with mean age 55.4. Fifteen sinuses lift with simultaneously 40 dental implants placed in augmented sinus, 20 implants placed in bone augmented sinus, other 20 implants in graftless sinus. Among the augmented sinuses 7 grafted and 8 without grafted bone materials, 3 sinuses used L-PRF without bone grafted in thickness Schneiderian membrane which potential for sinusitis complication. The lateral window shift to inner site as tenting to avoid membrane drop down. For grafted site, filled with Bio-Oss to supported membrane, the non-grafted site used collagen membrane and implant supported the membrane. The dental implants length insertion from 10 mm to 13 mm in mean residual bone height 4.3 mm. The lateral window cover with resorbable membrane with fixation pin and primary closure the wound. Submerged healed 6 months, then uncover the implants. All implant restorations via provisional restoration observation occlusion comfortable and stable then definitive prostheses delivery. The observation evaluations through CBCT measurement. All of implants firm primary stability were achieved, giving implants healed above 6 months. In grafting group the primitive bone significantly greater than graftless group (3.57 mm vs. 5.03 mm), and significantly bone gain when initial implant placement (11.28 mm vs.7.78 mm). But after 5 year bone remodeling, the augmented bone height no significantly difference. It indicate that grafting group and graftless group bone height gain the same, except the apex area, the grafting more larger than graftless group. Marked bone formation around implants observation with CBCT. One implant failed during provisional prosthesis stage, giving the survival rate of 97.5%. It is suggested that used non-grafted procedure in sinus augmentation may be an alternative treatment options.
