In western countries, including the United States, foreign-trained nurses constitute a crucial labor supply. Far and away the largest number of these nurses come from the Philippines. Why is it that a developing nation with a comparatively greater need for trained medical professionals sends so many of its nurses to work in wealthier countries? Catherine Ceniza Choy engages this question through an examination of the unique relationship between the professionalization of nursing and the twentieth-century migration of Filipinos to the United States. The first book-length study of the history of Filipino nurses in the United States, Empire of Care brings to the fore the complicated connections among nursing, American colonialism, and the racialization of Filipinos. Choy conducted extensive interviews with Filipino nurses in New York City and spoke with leading Filipino nurses across the United States. She combines their perspectives with various others—including those of Philippine and American government and health officials—to demonstrate how the desire of Filipino nurses to migrate abroad cannot be reduced to economic logic, but must instead be understood as a fundamentally transnational process. She argues that the origins of Filipino nurse migrations do not lie in the Philippines' independence in 1946 or the relaxation of U.S. immigration rules in 1965, but rather in the creation of an Americanized hospital training system during the period of early-twentieth-century colonial rule. Choy challenges celebratory narratives regarding professional migrants’ mobility by analyzing the scapegoating of Filipino nurses during difficult political times, the absence of professional solidarity between Filipino and American nurses, and the exploitation of foreign-trained nurses through temporary work visas. She shows how the culture of American imperialism persists today, continuing to shape the reception of Filipino nurses in the United States.

In this dissertation, I analyze how formal organizations and institutions construct an international market for labor and a corresponding industry for migration-related services. Focusing on the migration industry, I scrutinize the sector that is comprised of private recruitment and placement agencies and its intervening role in the market for foreign-educated nurses. Much of the literature on international migration focuses on the role of informal social networks in furthering international labor flows. My study, however, brings in insight from economic sociology and organizational studies to show that the movement of international labor can also be understood as a market, comprised of several organizational fields that profoundly shape the recruitment and migration of workers. I thus show that intermediary organizations provide alternative solutions to resolving market challenges (e.g. property rights, governance structures, and rules of exchange) where other social structures, such as informal social networks, may not or cannot. The dynamic relationship between formal organizations, institutions, social networks, and migration is important to model as today's migration processes are becoming increasingly complex and bound together with business interests. These conclusions are based on two years of fieldwork in the Philippines and United States, during which I conducted 98 in-depth interviews with nurses and leading representatives of several organizations and institutions. These organizations included nursing schools, sending and receiving governments, commercial recruitment and placement agencies, and employing hospitals. This research design allowed me to not only trace the social processes of the migration from origin to destination, but also to examine the migration system at different levels of analysis: from the individual decision making of nurses, to the meso-level organization of formal intermediaries and brokers in the system, and finally to the macro-level involvement of state institutions.

The Filipino American population in the U.S. is expected to reach more than two million by the next century. Yet many Filipino Americans contend that years of formal and covert exclusion from mainstream political, social, and economic institutuions of the basis of their race have perpetuated racist stereotypes about them, ignored their colonial and immigration history, and prevented them from becoming fully recognized citizens of the nation. Locating Filipino Americans shows how Filipino Americans counter exclusion by actively engaging in alternative practices of community building. Locating Filipino Americans, an ethnographic study of Filipino American communities in Los Angeles and San Diego, presents a multi-disciplinary cultural analysis of the relationship between ethnic identiy and social space. Author Rick Bonus argues that alternative community spaces enable Filipino Americans to respond to and resist the ways in which the larger society has historically and institutionally rendered them invisible, silenced, and racialized. centers, and the community newspapers to demonstrate how ethnic identities are publicly constituted and communities are transformed. Delineating the spaces formed by diasporic consciousness, Bonus shows how community members appropriate elements from their former homeland and from their new settlements in ways defined by their critical stances against racism, homogenization, complete assimilation, and exclusionary citizenship. Every year thousands of foreign-born Filipino and Indian nurses immigrate to the United States. Despite being well trained and desperately needed, they enter the country at a time, not unlike the past, when the American social and political climate is once again increasingly unwelcoming to them as immigrants. Drawing on rich ethnographic and survey data, collected over a four-year period, this study explores the role Catholicism plays in shaping the professional and community lives of foreign-born Filipino and Indian American nurses in the face of these challenges, while working at a Veterans hospital. Their stories provide unique insights into the often-unseen roles race, religion and gender play in the daily lives of new immigrants employed in American healthcare. In many ways, these nurses find themselves foreign in more ways than just their nativity. Seeing nursing as a religious calling, they care for their patients, both at the hospital and in the wider community, with a sense of divine purpose but must also confront the cultural tensions and disconnects between how they were raised and trained in another country and the legal separation of church and state. How they cope with and engage these tensions and disconnects plays an important role in not only shaping how they see themselves as Catholic nurses but their place in the new American story.

This is a qualitative study about cross cultural, as well as family conflicts that affect Filipino Nurse Migrants as they immigrate to the United States to work as nurses for different institutions. To report and document the conflict narrative of Filipino Migrant Nurses in the United States from the standpoint of the Nurses themselves. The ultimate goal of this project is to build a program of conflict prevention embedded in the recruitment of Nurses from the Philippines. 12 Migrant Families, 1 official of national organizations and 1 government official participated in the completion of this project between the dates of winter of 2004 to the summer of 2006. The narratives were heard and their personal, data audio recorded in the interviews. The interviews were then transcribed and the text analyzed. The focus of this analysis is the different conditions and experiences of the families and the specific, culture consonant ways in which a Filipino Migrant Nurse copes with a new environment. The study also concentrates on the different conditions and policies that prevail amidst shortage of nurses and the recruitment of nurses in the Philippines. Research suggests that the current recruitment process has led to worker overload amidst the nursing shortage and wage abuses as new recruits wait for their immigration documents. The study traces the historical and personal conditions that led Filipino Nurses to the choice of migration. This information is also triangulated between the different members of their partners and family members. The findings suggests that although various family conflicts are commonly experienced by migrant nurses as they move to the United States, there are very few institutions, private companies and hospitals addressing adjustment issues and related conflicts. In the midst of a nursing shortage, there are significantly more elements of migration than can be addressed by the current national security centered immigration approach. Institutional coordination and sensitivity to internal and external conditions from the different private and government agencies in both the Philippines and the United States is due. But before solutions are put forward, this study examines what adjustment issues Filipino nurses encounter as they have historically immigrated to the United States. Before any price can be put on their service, it would only be prudent to examine what price they pay in the process.
