Provides the practical tools, advice, and encouragement needed to help district and provincial health management teams tackle - and overcome - problems encountered in their daily work. Addressed to facilitators, the book concentrates on the use of three carefully planned and timed workshops as a framework for identifying and analyzing problems, developing practical solutions, and preparing and subsequently revising plans of action in the light of experience gained. The tools and techniques used at the workshops were developed and refined during two years of testing in the field. The manual has three parts. The first, which provides an overview of the learning process detailed in the rest of the book, uses a question-and-answer format to address a wide range of questions the reader may have concerning why the process is needed and what it can achieve. The second part sets out operational guidelines for planning, facilitation, and support. The third part, which constitutes the core of the manual, provides a detailed day-to-day guide to the scheduling of three separate workshops, the topics to be covered in each daily session, and the methods most likely to facilitate group participation and problem-based learning. The practical value of this material is greatly enhanced through the inclusion of a full set of model handouts to be used, after adaptation to the local situation, by participants.

Furthermore, they state that spending on health would not alone achieve the intended results in health without addressing governance. The District Health Administration (DHA), generally called the office of the Civil Surgeon in Bihar, India, is considered a bridging administrative unit from national and state government to the community at village level. Moreover, this is a full-bodied unit with management and the administrative power under the leadership of Chief Medical Officer/Civil Surgeon and Assistant Chief Medical Officer with various health programme officers and support staff. Although health service delivery is seen to have improved in Bihar in recent years, issues such as leadership, management, accountability and transparency would seen to be weak in the DHA. This study aims to recommend specific strategies to strengthen governance of the DHA in Bihar. In doing so, it first identifies and critically analyses the causes of poor governance of the DHA. Moreover, it reviews theories and documented effective examples of strengthening governance in the district level health systems in low and middle income countries. This study finally analyses various strategies for strengthening governance and outlines appropriate governance for the DHA in Bihar.To undertake this study, a framework for strengthening governance developed by Brinkerhoff et al. (2008) is used, which covers leadership and management capacity, accountability, community participation and transparency in decision making. The methodology primarily involves reviewing secondary data; however, a number of health and development professionals and legal experts were contacted to obtain a number of practical experiences.

Worldwide more and more governments have begun to buckle under a variety of strains, including the ongoing pressures of economic crisis, followed by structural adjustment programmes, and the impact of declining legitimacy, often resulting in the outbreak of civil war. In this study of aid policy, Joanna Macrae argues that the disintegration of state authority and civil order has created acute problems in aid management. Largely ignored by major aid organizations, insecurity and failures of governance are now the major obstacles to aid reaching those in most need. International aid has traditionally assumed the existence of stable, sovereign states capable of making policy. In a number of developing countries, including post-conflict regimes like Cambodia, Uganda or Kosovo, this is no longer the case. The big donor agencies have usually responded by suspending development aid and substituting some kind of emergency or relief assistance. Now, as the author shows, there are calls to make relief more development-oriented and for it to address the underlying conflicts which causes these crises. But she concludes from her investigations on the ground in a number of countries that relief and development aid are very distinct processes. In the absence of public policy-making authorities, aid becomes highly fragmented, often inadequate in scale, and certainly not capable of building local sustainability for particular programmes. The international aid system, she concludes, faces real dilemmas and remains ill-equipped to respond to the peculiar challenges of quasi-statehood that characterize chronic political emergencies and their aftermath.
