Will nurse directed, patient specific, education improve patient outcomes and have a more powerful effect on patients ability to self-manage their health care? Or is generalized patient education sufficient? Strokes have decreased to the third leading contributor to deaths in the United States. Improved rates in stroke preventive deaths have been attributed to the early recognition of stroke symptoms and improved responses of individuals activating the emergency response system (National Stroke Association, 2015).While a positive trend is noted in reducing stroke related deaths, there is still more progress needed in the areas of stroke education. Evidenced based literatures suggest that patients and caregivers are often discharged with feelings of inadequacy, resulting in the inability to properly manage their care once they are home (Yonaty and Kitchie, 2012). Inadequate or fragmented stroke patient education has been attributed to unplanned hospital readmissions, poor patient outcomes, and even recurrent strokes (Xu, 2012). According to The Stroke Joint Commission (2014), stroke education should be relevant and specific to the patient's diagnosis and current conditions, while addressing primary and secondary stroke prevention. Some stroke patients may suffer from cognition deficits; therefore repetition is required to aid with memory and retention. Health care literacy must be addressed, as poor health literacy is attributed to reduced patient outcomes, (Sanders, Schnepel, Smotherman, and Livingwood, and others, 2014).

The Aspen Center for Patient Education (ACPE) is dedicated to the development of low-literacy patient education materials in both English and Spanish. Health literacy represents a major challenge to health care professionals with nearly half of the U.S. population at or below the ninth grade level. More than 20% of the U.S. population cannot read materials at the fourth or fifth grade level. JCAHO Patient and Family Education Standards specify that health care professionals consider their patient's literacy, education level, and language in providing health instruction. Working with a distinguished list of counsulting editors, ACPE editors create practical, hands-on patient education materials across the complete spectrum of health care at both the third and sisth grade reading level in both English and Spanish. A comprehensive reference volume, the Stroke Rehabilitation Patient Education Manual supplies patient and practitioner information spanning the stages of stroke rehabilitation, the many risk factors, prevention of recurring strokes, and various treatments. This manual contains charts, guidelines, instructions, procedures, and other tools to aid in rehabilitation and facilitate patient compliance.
