Excerpt from Studie Uber Minderwertigkeit von Organen Da ein derartiger Zustand fur viele Falle anzunehmen ist und in der Nierenpathologie seine Rolle spielt ist wohl allgemein anerkannt. Die Erscheinung genuiner Nierenerkrankungen kann durch die An nahme hypothetischer Stoffwechselgifte nicht zureichend erklart werden. Vor allem ist es der pathologische Befund sowie der klinische Verlauf, die beide dem Bilde einer chronischen Vergiftung widersprechen. Fast mit der gleichen Scharfe wird diese Annahme durch die oft lange Dauer und durch das haufig hereditare Auftreten widerlegt. Ebenso sind die Pu bertatsalbuminurie, die renale Hamophilie, die Zystenniere, die Schwan gerschaftsniere, die orthostatische Albuminurie und die mit chronischer Obstipation verbundene Albuminurie Hinweise denen man sich kaum entziehen kann. Eine der machtigsten Stutzen aber fur die den Nieren erkrankungen zugrunde liegende Minderwertigkeit des harnabsondernden Apparates ist die in der Nierenpathologie so haufig zu beobachtende Hereditat. Hochstens kommt noch in Frage ob einzelne dieser Albu minarian als Krankheiten zu nehmen sind. Die Schwierigkeiten in der Entscheidung dieser Frage sollen nicht geleugnet werden. Denn der Ubergang von leichten Anomalien der Harnbeschati'enheit zu schweren Erkrankungsformen der Nieren ist noch nicht haufig genug beobachtet worden. Aber selbst langjahriger Stillstand der Erscheinungen oder Besserung, sei sie unter arztlicher Behandlung oder ohne eine solche erfolgt, hat keinerlei Beweiskraft. Es kann namlich mit Recht hervor gehoben werden, da die Annahme einer Minderwertigkeit der Niere als Grundlage der Nierenerkrankungen vom Standpunkte der Patho logie aus betrachtet, sehr viel Wahrscheinlichkeit fur sich hat, da der Ubergang von Bildungs oder Funktionsanomalie in Krankheit in der kurzesten Frist bewerkstelligt sein kann und es in vielen Fallen frag lich wird, wo fur uns das Krankheitsbild beginnt. Die "physiologischen Albuminurien spielen da die gleiche Rolle wie etwa die Zystenniere, die sich formlich uber Nacht als schwerer Krankheitsfall darstellen kann, nachdem sie langere Zeit symptomlos bestanden hat.
