With recent studies using genetic, epigenetic, and other molecular and neurochemical approaches, a new era has begun in understanding pathophysiology of suicide. Emerging evidence suggests that neurobiological factors are not only critical in providing potential risk factors but also provide a promising approach to develop more effective treatment and prevention strategies. Suicide is the third leading cause of death among adolescents in the United States. Despite its prevalence, research on adolescent suicide has been largely disjointed and theoretically ungrounded. The purpose of this study was to test a proposed model of adolescent suicide guided by the Interpersonal Psychological Theory (IPT) and existing empirical evidence. Structural equation modeling was used to longitudinally investigate the underlying structure of the IPT constructs of thwarted belongingness and the acquired capability to self-injure and investigate proposed pathways of risk among a large group of urban adolescents. Results supported a multidimensional structure of the IPT constructs. Only two constructs predicted suicidal ideation over time: internalizing behavior problems and substance use. Further, internalizing behavior problems mediated the relationship between substance use in early adolescence and suicidal ideation in late adolescence. Results inform prevention and intervention efforts by emphasizing the role of distal risk factors, especially as related to early substance use and mood symptomatology.
