Objective: To explore the impact educational debt has on our quality of life, well being, and career and life decisions we make as pediatric dentists. Methods: A web-based survey was designed, piloted with Nationwide Children's Hospital pediatric dental residents and selected faculty, and approved by the Institutional Review Board of Nationwide Children's Hospital. The survey was distributed via the American Academy of Pediatric Dentistry membership email list to assess demographics, educational indebtedness, income profile, and impact of debt on career and life decisions, stress and well being. Results: 1171 US pediatric dentists or pediatric dental residents responded (20% response rate). No post-residency debt was reported by 169 (14%), nine hundred ninety-five reported post residency debt (85%), and eleven did not respond (1%). Of those reporting post-residency debt, about 22% reported debt > $300,000. Thirty-two respondents do not currently earn income, and of those who do earn income, about 59% earn = $200,000. About 30% of respondents are not currently paying toward educational debt, while 5% are paying = 40% of their salary toward educational debt. About 60% of respondents reported that their debt had influence on their choice to own a practice or be an associate. Between 60-70% reported that their debt had influence on work life balance, hours worked, and ability to buy a home. Sixty percent agree or strongly agree that they experience stress related to personal finances, 40% report experiencing a medium to extreme amount of stress as a results of student loans, and 60% report experiencing a medium to extreme amount of stress as a result of total debt. Ten percent reported that they almost always have trouble relaxing because of their debt. Amount of indebtedness after residency and percent of annual income paid toward debt are strongly related (Spearman Correlation Coefficient of 0.70). Both post-residency debt and income paid toward debt are moderately correlated with nearly every variable related to career choice, and the degree of influence debt has on life choices (for all decisions except the choice to have a committed relationship). For all career decisions examined, either level of debt or percent of income paid toward debt (or both) is a significant predictor of odds that debt influences decisions. For all life decisions examined, post-residency debt is a significant predictor of all decisions, while income paid toward debt was a significant predictor for the following decisions: having children, volunteerism, purchase of home, and hours worked. Conclusion: Our percent of income toward debt trends down reliably over time and does not appear to affect our self-esteem or optimism regarding the future. Our debt is significantly impacting our stress levels, weighing much more heavily on the newly graduated, and playing a role in our decision making processes both personally and professionally. The higher the debt, the greater the influence of the debt in all areas.
