Considering the detrimental environmental impact of current food systems, and the concerns raised about their sustainability, there is an urgent need to promote diets that are healthy and have low environmental impacts. These diets also need to be socio-culturally acceptable and economically accessible for all. Acknowledging the existence of diverging views on the concepts of sustainable diets and healthy diets, countries have requested guidance from the Food and Agriculture Organization of the United Nations (FAO) and the World Health Organization (WHO) on what constitutes sustainable healthy diets. These guiding principles take a holistic approach to diets; they consider international nutrition recommendations; the environmental cost of food production and consumption; and the adaptability to local social, cultural and economic contexts. This publication aims to support the efforts of countries as they work to transform food systems to deliver on sustainable healthy diets, contributing to the achievement of the SDGs at country level, especially Goals 1 (No Poverty), 2 (Zero Hunger), 3 (Good Health and Well-Being), 4 (Quality Education), 5 (Gender Equality) and 12 (Responsible Consumption and Production) and 13 (Climate Action).

'Why we eat what we eat?' is a key question for the 1990s, posed again and again in government departments, in sectors of the food industry, by professionals in health, in education, and in catering, to name a few. It is the same question adopted as the springboard for the UK Economic and Social Research Council's (ESRC) Research Programme on 'The Nation's Diet' (1992-1998), a wide ranging, multi-disciplinary set of co-ordinated basic research projects across the social sciences, including economics, psychology, social anthropology and sociology, as well as education and media studies. Traditional foods, diets and food systems play an important role in people’s nutritional status. As the best products of traditional food systems, geographical indication (GI) foods have a great potential to contribute to healthy diets and curb non-communicable diseases. This paper presents five case studies around the world on the nutritional potential of registered GI foods and explores the link between the production processes and the nutritional composition of the final products. The nutritional characteristics of these foods can be largely attributed to their unique ingredients and production procedures, which are linked to their geographical origins. The paper also discusses the development of GI specifications to maintain and improve nutritional values, the role of GI foods in healthy diets, and the determination of food composition. Finally, suggestions on how to leverage GI foods for healthy diets are provided.

Monitoring of dietary intake at national and global levels is becoming increasingly important with the changing food systems and diets. Better measurement and monitoring are needed to support governments in establishing policies and programmes to promote healthy diets and assess the effectiveness of these actions. However, there is lack of consensus on the indicators that best capture the properties of healthy diets. Recognizing the need for consensus and action, the Food and Agriculture Organization of the United Nations (FAO), the United Nations Children’s Fund (UNICEF), and the World Health Organization (WHO), have joined forces to chart a way forward via Healthy Diets Monitoring Initiative (HDMI). This is the first version of the guidance for monitoring healthy diets globally, which provides an overview of healthy diets measurement purposes, types of surveys suitable for the monitoring purpose, appropriate dietary assessment methods, relevant dietary data types and potential healthy diet metrics for national and global monitoring purposes. The guidance will be enhanced in the coming years to reflect the most up to date technical and operational recommendations, after rounds of consultations with countries and experts actively engaged in the implementation of surveys and data collection processes.

This report consists of two main sections: 1) a situation analysis of Sustainable Development Goal 2 Target 2.1 (to end hunger and ensure access to food by all) and Target 2.2 (to end all forms of malnutrition) and analyses of the diets of children and of current food consumption patterns relative to dietary guidelines; and 2) a special look at the cost and affordability of healthy diets in Europe and Central Asia. The new estimates confirm that the prevalence of hunger at chronic or severe levels is relatively low in the ECA region. However, the prevalence of food insecurity at moderate or severe levels can be quite high. The ongoing COVID-19 pandemic continues to add people to the ranks of the food insecure. The ECA region is making progress overall in reducing malnutrition, but it is not on track regarding childhood overweight, adult obesity, anaemia and exclusive breastfeeding. Healthy diets protect against the effects of malnutrition, in all its forms, and promote better health. This report shows that healthy diets cost, on average, five times more than diets that meet only dietary energy needs, making them unaffordable for many people throughout the region. To increase the affordability of healthy diets, the costs of nutritious foods must be lowered. This report also shows that hidden costs are much lower with healthy diets than with current consumption patterns, meaning that adopting healthy diet alternatives could lead to large cost savings.

Diet and nutrition are critical to health, well-being and longevity. The economic and health burdens associated with poor quality diets are a worldwide concern, but for low- and middle-income countries (LMICs), the long-term impact of these burdens has the potential to be especially devastating. Many of these LMICs are currently grappling with the deepening multiple burdens of malnutrition, with undernutrition, nutrient inadequacies, and overweight and obesity often presenting simultaneously in communities, households and even in single individuals. Time-relevant data is a necessary and critical component of any process or initiative that aims to ensure healthy diets. Robust data on what people eat in a country enables an understanding of current food consumption practices, and provides an evidence-based foundation for the design and implementation of targeted and well informed actions, policies and messaging to address the key issues related to healthy eating. The purpose of this report is to take stock and celebrate the collection and use of dietary data in LMICs, and generate further momentum for investment in government-led dietary surveys in LMICs. Section 1 provides a global overview of dietary surveys carried out in LMICs from 1980 through 2019, analysing key characteristics and trends over time. Section 2 celebrates the increased investment in dietary surveys in LMICs by highlighting country stories related to dietary survey initiation, implementation and data use. Section 3 is aimed at generating further momentum for investment in government-led dietary surveys in LMICs by illustrating, through data visualizations, the type of information dietary data can provide for policy makers.
