In many respects, the continent of Africa is in transition. Prominent among them – currently – is the nutrition transition. One consequence of the nutrition transition is the increase in prevalence of nutrition-related non-communicable diseases (NCDs) such as obesity, type 2 diabetes, cardiovascular diseases, and certain cancers. Although NCDs are a global public health problem, the rate of increase in NCDs morbidity and mortality in some African countries is staggering. This surge has been linked to modifiable environmental factors – factors that facilitate the consumption of obesogenic (energy-dense nutrient-poor foods), rather than unrefined cereals, fruits, and vegetables. It has long been recognized that the physical and social environments - in which people live, work, and eat are critical determinants of their health. More recently, there has been a greater focus on the food environment as a key determinant of health. Available evidence shows that unhealthy food environments drive unhealthy diets; and unhealthy diet is one of four main risk factors for NCDs.

Hunger, malnutrition, poor health, and deficient food systems are widespread in Sub-Saharan Africa. While much is known about African food systems and about African health and nutrition, our understanding of the interaction between food systems and health and nutrition is deficient. Moreover, the potential health gains from changes in the food system are frequently overlooked in policy design and implementation.The authors of The African Food System and its Interactions with Human Health and Nutrition examine how public policy and research aimed at the food system and its interaction with human health and nutrition can improve the well-being of Africans and help achieve the United Nations Millennium Development Goals (MDGs). Urban population growth is extremely rapid across Africa and this book places urban food and nutrition security firmly on the development and policy agenda. It shows that current efforts to address food poverty in Africa that focus entirely on small-scale farmers, to the exclusion of broader socio-economic and infrastructural approaches, are misplaced and will remain largely ineffective in ameliorating food and nutrition insecurity for the majority of Africans. Using original data from the African Food Security Urban Network’s (AFSUN) extensive database it is demonstrated that the primary food security challenge for urban households is access to food. Already linked into global food systems and value chains, Africa’s supply of food is not necessarily in jeopardy. Rather, the widespread poverty and informal urban fabric that characterizes Africa’s emerging cities impinge directly on households’ capacity to access food that is readily available. Through the analysis of empirical data collected from 6,500 households in eleven cities in nine countries in Southern Africa, the authors identify the complexity of factors and dynamics that create the circumstances of widespread food and nutrition insecurity under which urban citizens live. They also provide useful policy approaches to address these conditions that currently thwart the latent development potential of Africa’s expanding urban population.

The local urban and home-food environments of white adults living in the Eastern suburbs of the Tshwane Metropole is explored and described. The study focused on the contribution of these environments to the food practices of the study group. The South African population, similar to those of other developing and sub-Saharan countries, is undergoing rapid urbanisation that is closely associated with a number of social structural changes such as migration, modernisation, globalisation, economic advancement and acculturation. As a result food systems and environments simultaneously undergo major changes due to technological advancements, food policies and lifestyle changes of consumers. The urban environment further seems to negatively influence the food patterns of the urban South African population as the majority follow a pattern associated with high intakes of energy, salt, saturated fat and refined sugars with a low intake of fruit, vegetables and fibre. Currently much attention focuses on how the urban food environment influences and shapes consumers' food intake and subsequent health. As there is limited information on the eating patterns and food-related behaviour of white South African adults, this study fills a gap in the literature. A quantitative research approach is followed in this cross-sectional study to achieve the objectives of the research. An electronic survey questionnaire, consisting of closed and open-ended questions was developed. Information gathered concerned the current eating patterns of the respondents, including aspects related to the local urban food and home-food environments. A total of 230 white adults in Regions 3, 4 and 6 of the Tshwane Metropolis completed the questionnaire. The white adults in Tshwane follow a typical Western eating pattern. The meal patterns consisted of 3 meals a day with in-between snacking. Most respondents eat all their meals at home and the majority consistently eat breakfast every day. Some eat meals away from home daily, either at the workplace or at restaurants. Although the majority consumed a variety of food daily as reflected in the Dietary Diversity Score of 6.20, a low consumption of legumes, fruit and vegetables by some respondents, raises concerns. Urban consumers in Tshwane have easy, adequate access and transport to most food stores. Supermarkets are frequently visited for food purchases and the majority do so at least once a week. The respondents were satisfied with the range of food outlets; including the quality and variety of foods, they have access to in their neighbourhoods. This study group eats family meals together on a daily basis and regarded their homes as places where healthy family meals were prepared and enjoyed. Positive attitudes towards healthy eating and family meals confirmed these findings. This study is significant and valuable and can assist in the development of intervention strategies to promote healthier food choices and further improve the food practices of urban consumers. Recommendations from the results of this study suggest that consumer educators and facilitators should thus target consumer education towards those Food-based Dietary Guidelines for South Africans that do not seem to be followed in the daily food practices of the study group.

Food environments in Africa are changing rapidly, with modern retailers - such as supermarkets, hypermarkets, and fast-food restaurants - gaining in importance. Changing food environments can influence consumer food choices and dietary patterns. Recent research suggested that the growth of supermarkets leads to more consumption of processed foods, less healthy diets, and rising obesity. However, the use of modern retailers may differ by socioeconomic status, which was hardly considered in previous work. Furthermore, existing studies on nutrition effects focused mainly on the role of supermarkets, although most consumers obtain their food from various sources. We add to this research direction by examining more explicitly the relationships between socioeconomic status, use of different modern and traditional retailers, and dietary patterns. The analysis uses household survey data from urban Zambia. Results show that twothirds of the households use modern and traditional retailers simultaneously, whereby richer households are more likely than poorer households to use supermarkets and hypermarkets. Use of modern retailers is positively associated with higher consumption of ultra-processed foods, also after controlling for income and other socioeconomic factors. However, the use of traditional grocery stores and kiosks is also positively associated with the consumption of ultra-processed foods, suggesting that modern retailers are not the only drivers of dietary transitions.
