Unconditional positive regard is the therapist's attitude of acceptance of the feelings the client expresses in therapy. Unconditional positive regard results from the therapist's nondirective responses to the client, such as attentive listening and emphatic restatement. However, therapists have reported in studies that different diagnostic categories of clients may not be able to respond to or understand nondirective behaviors as conveying the therapists' unconditional positive regard. Therapists have also pointed out in studies examples of when clients may misunderstand the therapists' nondirective behavior as not being unconditional positive regard. Further, a body of literature that investigates how therapists convey their attitudes to clients indicates that it is difficult to determine exactly how this occurs. This case study examined the application of unconditional positive regard in the practice of psychotherapy. The central problem of interest is stated in the question: Could the therapist convey unconditional positive regard by responses that were other than nondirective? Other issues related to the central question are also addressed. For example, one such question asks: What is the relationship between uncondition positive regard and resistance, particularly is unconditional positive regard effective in addressing the resistance of the client? Aspects of the client-therapist relationship are also discussed in terms of the central question. The author reports the case material on a session by session basis. He places emphasis on the development of the client-therapist relationship in context of unconditional positive regard and how it is conveyed to the client. He has recreated dialogue based on the extensive notes and recordings of the sessions he made. This study suggests that the therapist can convey unconditional positive regard through an eclectic approach which includes nondirective behaviors. It further suggests that the therapist can effectively address resistance using an eclectic approach that conveys unconditional positive regard. It suggests modifications of the client-centered model, particularly a modification of the underlying assumption of the actualizing tendency that would include developmental, social, and environmental factors, as well as, the aspect of an inherent tendency toward growth.
