Excerpt from The Archives of Internal Medicine, Vol. 16: October 15, 1915 It is not within the scope of this paper to include more than a brief summary of the literature. A complete bibliography of those publica tions which specifically consider the relation between syphilis and Raynaud's disease has been added for those more fully interested. Raynaud's disease is uncommon, yet its occurrence is not so extremelv infrequent as to relegate it to the category of medical rarities. Cassirer saw 56 cases, and including a careful review of the literature up to 1911, has gathered together over 300 unquestioned instances of the disease. Coming now to the group of cases in which an hereditary or acquired lues is present, we find a considerable discrepancy in the figures published. Monro would allow only per cent, while Cas tellino and Cardi find 22 cases of syphilis in 306 cases of Raynaud's disease. Probably the proportion is still higher. The appended 54 references contain more than 22 cases. Perhaps 10 per cent. Will not be too high an estimate. Cannon, Erlanger, Hewlett, Hirschfelder, Moffitt and Wilbur returned the photographs with their marks; and these marks have all been transferred to the original photographic negatives which are herewith reproduced in Figures 1 to 6. Connected with each mark is a letter (arbitrarily assigned), which represents the one who chose that point for his reading, and a number, which shows in millimeters the displacement of mercury in the U-tube at the instant when this point was written. These numbers were Obtained by reference to the original drum records, which contain tracings Of a float in the mercury U-tube, together with its base line and, at the beginning and end Of each record, stationary marks. A mail-carrier in the rural free delivery service, having to ride thirty miles daily over rough roads. There was some exposure to cold. But not excessive. Four years ago, in the spring, he first noticed frequent urination, both diurnal and nocturnal. This rapidly increased in frequency, until it became embarrassing. 110 would have to st0p every half hour or even more frequently while on his route. Auv attempt to suppress this tendency resulted in great misery. The quantities were large at each urination. There would be four or five nocturnal micturitions. The quantity was several quarts a day. His mouth and throat felt constantly parched, in spite of the great amounts of water drunk. The tongue was dry. Bulimia also developed in the spring. Excerpt from The Archives of Internal Medicine, 1913, Vol. 11 When we consider the medical history of Panama with reference to the origin of its population, the negroes from Africa and possibly the Pacific Islands, Chinese, introduced by the French Canal Company; Coolies, Sikhs and Bengalis from India, besides those representatives of the Caucasian race who have come from every portion of the tropical and subtropical world, Americans from the Philippines, various globe-trotters and tropical tramps, we naturally expect to encounter here some of the diseases especially characteristic of other lands, and we are not surprised to find that echinococcus infections are confined strictly to Spanish laborers. Infection by Taem'a mum and T. Saginata occurs among Europeans and Americans. Bilharziosis comes almost exclusively from the French West India Islands, Filaria demarquai in negroes from St. Vincent, Old World hookworms, A. Duodenale, among the West Indians, possibly introduced there by coolies. Trichinosis has been detected only among the Spanish (from Spain). Some of these diseases are absolutely confined to the races or nation alities mentioned, because the local environment here is quite unsuited for their transmission to the native population. For example, there is no instance of any kind of tapeworm or fluke infection among the native population. It is otherwise, however, with hookworm disease. For the Old World worm is now found in many of the West Indians and in some Panamanians. If a sailor infected with aestivo-autumnal malaria from the west coast of Africa should have a sufficient number of gametes in his peripheral blood on arriving here, and be bitten by Anopheles albimanus, 60 per cent. Of the mosquitoes would become infected and might later transmit the disease to others the secondary cases would clinically conform to the local type of malaria, and there would be no difference in morphology between the plasmodia and those in cases seen here. It is, however, a resultant of the two preceding components; that is, of the average blood flow and of the variations in this flow produced by the entrance of the pulse wave into the arm.
