Restorative dentistry has experienced a shift from the mainly reparative dentistry of the 20th century towards a minimal intervention approach encompassing up to date caries diagnosis and risk assessment before arriving at treatment decision. A shift in philosophy from the traditional surgical model of excision to a more modern medical model of treating caries is occurring so, when operative care is indicated, it should be aimed at "prevention of extension" rather than "extension for prevention". This book emphasis on the minimal invasive concepts consisting of individual caries risk assessment, reduction of cariogenic bacteria, early caries diagnosis, remineralization of non-cavitated arrested lesions, minimal tooth preparation with modified cavity designs and repair rather than replacement of the restoration. Also minimal invasive techniques including air abrasion, atraumatic restorative technique, chemomechanical methods, tunnel restorations and lasers for treatment of dental caries have been discussed. We believe that this book will provide valuable knowledge to students and practitioners in applying minimal intervention concepts, thus preserving the natural tooth structure.

Currently there is no reason, in most cases of cavitated caries lesions, to remove affected tissue. This book presents evidence-based research on the topic and provides assessments of diagnostic devices. It offers new insights into how a dentine carious cavity can be managed by either tissue removal or restoration. Methods for preserving dental tissue are presented and ample evidence highlights the need to seal with a quality restorative material. An update on how to conduct a randomized clinical trial is followed by a chapter on agreed upon terminology for supporting improved communication among oral health professionals around the world. This is a must-read for general practitioners, restorative specialists, dental students, and oral hygienists/therapists.

The aim is to provide practitioners with thorough, up-to-date information that will improve their clinical practice. The opening chapters present a comprehensive overview regarding diagnosis of carious lesions, risk assessment, child behavior and development, and behavioral management. The importance of oral health promotion and prevention in controlling lesion progression and maintaining oral health is reviewed. The impact of various factors on clinician decision making is then explained in detail, examples including the type of dentition (primary versus permanent), the clinical and radiographic aspect of the dentine carious lesion (noncavitated or cavitated), and whether the lesion is associated with a developmental defect. Guidance is provided on selection of nonoperative versus operative interventions, and the restorative materials most frequently used in pediatric dentistry are fully described, highlighting their advantages and disadvantages. Readers will also find an informative series of cases, with explanation of the choices in terms of materials and approach.
