Even though progesterone/progestogen is not licensed for this purpose in the UK, it is commonly prescribed in many countries. The guideline examines the evidence for the risks and benefits of this treatment. The clinical and cost effectiveness of expectant, surgical and medical management for miscarriage and surgical and medical treatment of ectopic pregnancy are considered, with reviews looking at both the risks and benefits of each strategy in terms of clinical and psychological outcomes. Cost effectiveness is an extremely important component of any guideline, in order to ensure that the limited resources of the National Health Service are used to maximise health benefits for its users. The final advice and selection of first line treatment takes this into account. The guideline does not cover pregnancy after the first trimester (after 12 completed weeks of pregnancy). It also does not deal with unusual conditions that present with pain and bleeding, such as hydatidiform mole, which require a different form of treatment. Similarly, it does not consider recurrent miscarriage, as this requires more specific investigation and management.

"Objectives: While medical management has been used to treat early pregnancy loss for over two decades, there remains a paucity of reliable predictors of treatment efficacy. The principal goal of this thesis is to identify new factors influencing medical management failure in women with first trimester pregnancy loss. Two objectives will be discussed. The first aim is to assess the effect of menstrual gestational age on the efficacy of misoprostol, in a population of women with a low sonographic gestational age. The second objective is to assess the value of uterine content ultrasound (US) measurements in predicting medical management failure in women with early pregnancy loss.Research design and methods: Two retrospective observational studies were conducted, each corresponding to a thesis objective. Both studies are based on a cohort of women with early pregnancy loss having visited the Emergency Department (ED) of the Jewish General Hospital, Montreal, Canada, between 2011 and 2013. Only women having been discharged with an outpatient prescription for misoprostol were included in out studies. With respect to the first objective, women had to have an US gestational age below 8 weeks. Exposure of interest consisted of menstrual age. In regards to the second objective, all women with US imaging available for review were included. Exposure of interest was uterine content measurements including: content anteroposterior distance (CAPD), content longitudinal distance, content transverse distance and uterine content volume. For both studies, logistic regression was used to estimate the effect of exposure on failed medical management defined as need for dilatation and curettage (D&C) or unplanned return to the ED (URED). Results: Increasing menstrual age was associated with an increased risk of D&C and URED. Specifically, risk of D&C and URED was 13.64% at ≤ 8 weeks and 26.32% at > 8 weeks, p

The first few months of any pregnancy are of supreme importance to the success of that pregnancy. This statement is so obvious as to be almost a platitude, yet it must be said that no aspect of pregnancy has been more neglected in the human than the first three months. Little is known of the morphological changes that occur at that time and our knowledge of the mechanisms that control this vital stage of pregnancy is almost non-existent. The explanation for this neglect of what is an obvious area for study is the difficulty of obtaining normal material. It is rare to have material to study from a healthy first trimester pregnancy and the study by Hertig and Rock!l) of early conception found by chance in hysterectomy speci mens must be unique. The information that we do have about early pregnancy is mostly gained from animal studies or single miscarriages in humans. Chromosomal defects are common but are not an explanation for the majority of recurrent miscarriages. Obstetricians have hypothesised many causes for this condition and have deve loped numerous metQods for treating it, but the studies have been poorly con trolled so that our understanding of the cause(s) has not advanced. Treatment of women with a history of recurrent miscarriage by paternal leuco cyte infusion (immunotherapy) may be yet another form of treatment that is hailed as a new advance only to be rejected when subject to rigorous testing.
