This book addresses all aspects of white coat hypertension – the phenomenon of raised blood pressure in a medical setting yet not elsewhere – from its history to its pathophysiology, diagnosis and treatment. Abstract: Objectives Hypertension is increasingly found in children. 24 hour ambulatory blood pressure monitoring is now preferred to office blood pressure measurement for diagnosis and monitoring. Blood pressure of children tends to fluctuate widely throughout the day and night. A number of children suffer from a condition called white coat hypertension in which elevation of blood pressure is only transient and they usually have normal blood pressure on continuous monitoring. This condition is not totally benign. Some patients will evolve into hypertension over time. This retrospective case control study is to compare the demographic characteristics and blood investigations of these 2 groups namely white coat hypertension cases and a control group so as to identify factors predictive of white coat hypertension in a Chinese population to help allocate resources for timely diagnostic investigation. Method 100 patients referred from primary health care units aged 6-18 year old for suspected hypertension based on raised office blood pressure measurements were recruited as cases in the white coat hypertension group and 100 age-matched patients as a control group. The factors studied included gender, body mass index, length of gestation, birth weight, family history of hypertension, positive past medical history, blood pressure at first presentation, blood pressure of ambulatory blood pressure monitoring, serum glucose, urate and cholesterol and triglyceride level. Univariate analysis was used to detect significant difference between the cases and the controls for continuous and categorized variables. Logistic regression was used to assess the independent associations of significant predictive factors of white coat hypertension. Correlations with the scatterplots for the relation of office systolic blood pressure and diastolic blood pressure with 24 hour ambulatory blood pressure monitoring within cases was done to see if there was any significant relations. The outcome of cases was also studied for the proportion which progressed into genuine hypertension. Results Body mass index (BMI) was identified as a significant predictive factor in logistic regression with an odds ratio of 1.18 (95% CI = 1.11-1.25) with p-value Conclusion To conclude, this study showed that high BMI might be predictive of white coat hypertension in Chinese children in Hong Kong. This result could be act as a hypothesis for future research. Multi-centre prospective cohort studies might be of value in testing this factor together with factors which were shown to be significant in other studies. White coat hypertension is not always as benign as thought. Obesity is a condition which requires attention from patients, parents, paramedics and healthcare practitioners especially in primary healthcare settings and should be monitored more closely for the emergence of associated complications. Introduction: White coat hypertension is a phenomenon in which a patient's blood pressure in a clinic setting exceeds the patient's blood pressure in other settings. Studies suggest this occurs in anywhere from ten to thirty percent of patients. The identification of such is important as it may prevent unwarranted use of anti-hypertensive medications. Objectives: The main goal of this study was to quantify the prevalence of white coat hypertension at a free blood pressure clinic in Richmond, VA. The examination of blood pressure differences between clinic settings and other settings on a group and individual level was fundamental to this study. Further analyses were then included to examine risk factors associated with uncontrolled hypertension in attempt to develop a predictive model. Methods: Longitudinal data was collected from ambulatory blood pressure monitors of 16 patients from the Richmond Center for High Blood Pressure. This data was merged with data collected from retrospective chart reviews to obtain laboratory values for possible risk factors. Paired t-tests were used to examine differences between clinic and home measurements on a group level. The examination of individual agreement and regression analysis to develop a predictive model was also undertaken. Results: Twenty-five percent of subjects included in this study suffered from white coat hypertension. Overall, there was no difference between clinic and home blood pressure measurements on the group level. However, individual agreement was poor. Regression analysis showed high density lipoprotein was positively associated with and triglycerides were negatively associated with an increased percent of uncontrolled hypertension. Conclusion: White coat hypertension is a phenomenon that is present at the Richmond Center for High Blood Pressure. Recognition of this has the potential to be economically advantageous for the clinic. Due to a small sample size, results of this analysis are not considered predictive models. However, future research in this field would be served well to explore these preliminary findings with larger samples.

The introduction of ambulatory blood pressure monitoring in the 1960s provided new insights into the nature of high blood pressure disorders. Blood pressure is now categorised into four quadrants:normotension, masked hypertension, hypertension and white-coat hypertension. In white-coat hypertension blood pressure is elevated when taken at the doctor's office but normal if taken outside the doctor's office.Several controversies are associated with white-coat hypertension, which are discussed in Chapter 1. Whether white-coat hypertension is a condition of increased cardiovascular risk is the most important current issue. The consensus from prospective studies has been that the cardiovascular risk in white-coat hypertension is similar to normotensives, but recent studies suggest that white-coat hypertension is associated with an increased risk for the development of hypertension, which may mean an increased cardiovascular risk is present but delayed in white-coat hypertension. Examining the literature (Chapter 2)at the time of the thesis commencement highlighted inconsistencies in the definition for white-coat hypertension, which makes it difficult to determine if white-coat hypertension is associated with biomarkers of increased cardiovascular risk.This thesis by publication investigated whether white-coat hypertension is a condition of increased cardiovascular risk by determining: (1) if biomarkers of increased cardiovascular risk that are known to be present in essential hypertension are present in strictly defined white-coat hypertension based on aconsensus definition for white-coat hypertension (Chapter 4); (2) if white-coat hypertension is associated with an increased morning blood pressure surge (Chapter 5); (3) if white-coat hypertension is associated with increased risk for the development of new-onset hypertension; and (4) if biomarkers measured at baseline predict the development of sustained hypertension in white-coat hypertension (Chapter 7). Participants underwent measurement of artery stiffness, autonomic function, glucose and insulin status,circulating measures of inflammation and endothelial dysfunction, and twenty-four hour ambulatory blood pressure monitoring to confirm blood pressure status. White-coat hypertension participants required two twenty-four hour ambulatory blood pressure monitorings to confirm blood pressure status. Participants returned yearly for ambulatory blood pressure monitoring for three years. New-onset hypertension was defined based on elevated mean ambulatory blood pressure. This research has identified that white-coat hypertension is associated with markers of increased cardiovascular risk, including an elevated baseline two-hour glucose post glucose load and increased progression to sustained hypertension compared to normotensives. Elevated baseline measures of waist circumference, artery stiffness and two-hour load blood glucose were elevated in white-coat hypertension who progressed to sustained hypertension.White-coat hypertension was not associated with an increased morning blood pressure surge butunivariate analysis found lipids were associated with the morning blood pressure surge (Chapter 5), which was confirmed in a larger study that included normotensive, treated and untreated hypertensive participants (Chapter 6). White-coat hypertension should be considered a condition of increased cardiovascular risk. Based on the results of this thesis treating doctors are advised that they should not focus solely on the white-coat hypertensive subject's blood pressure but on their total cardiovascular risk and such subjects should be monitored for the development of both hypertension and type 2 diabetes.
