Rotavirus is the leading cause of acute gastroenteritis in children. In the United Kingdom, it causes significant short-term morbidity, and places a substantial burden on the health service. Two rotavirus vaccines have completed clinical trials and are being considered for routine infant vaccination. We investigated the cost-effectiveness of a universal immunisation programme in the United Kingdom, in order to inform the decision of whether to add one of the vaccines to the national vaccine schedule. A cohort model was constructed to compare the costs and outcomes of vaccination. Direct and indirect costs were considered separately. From the health care provider's perspective, the cost of rotavirus-related hospital admissions, nosocomial hospital infections, Accident and Emergency admissions, GP consultations and calls to a telephone health information service (NHS Direct) were considered. We also considered loss in quality adjusted life years (QALYs) for patients and their carers due to fatal and non-fatal episodes of rotavirus gastroenteritis. The economic value of lost productivity for those caring for children with rotavirus was also taken into consideration. Results of the model showed that the cost per QALY of an immunisation programme varied considerably depending on the perspective taken. Including QALY losses to carers had the effect of decreasing the cost effectiveness ratio by 56%, while including productivity losses in carers resulted in an additional 8% decrease. This has important implications for evaluating other childhood vaccines such as the pneumococcal conjugate vaccine. Similar results and influences were observed for other European countries, and will also be discussed.
