Abstract: Oral piercing is common, and mostly involves the tongue. Since gingival recession is of particular concern to the periodontist, the present cross sectional study aims to identify factors that contribute to the development of recession on the lingual of mandibular anterior teeth, in subjects with tongue piercing. Seventy-nine (79) subjects with a tongue piercing were recruited. Subjects were given a clinical exam on the mandibular anterior sextant (#22-27) and a questionnaire to obtain demographic and behavioral data. Fifty-seven (57) subjects had lingual recession present. Abstract: Mechanical trauma has been associated with detrimental effects of the periodontium and dentition; however, ethical considerations have prevented longitudinal prospective studies to examine factors associated with these effects. Tongue piercing, the most common intraoral piercing, has been associated with recession on the lingual aspect of mandibular anterior teeth and tooth chipping/fracture; yet, longitudinal studies have not been reported. The present longitudinal study of subjects with a recent tongue piercing aimed to identify anatomical and clinical factors associated with the development and progression of recession and tooth chipping/fracture. Thirty (30) healthy young adults (mean age = 19.6 years, age range 18-28; 21 females, 9 males) who had received a tongue piercing within the previous 5 months (mean time = 1.8 months, range = 0.25-5.0 months) were recruited in the study. Subjects were examined at 0 (baseline), 2, 6, 10, and 14 months. Probing depth (PD), recession depth (RD), plaque index (P1), and gingival index (GI) were recorded on the lingual (test) and buccal (control) aspect of six mandibular anterior teeth (#22-2 7). Tooth chipping/fracture were recorded for all teeth in the maxillary and mandibular arch. Subjects also completed a questionnaire to provide information related to the tongue piercing. Twenty two (22) subjects (mean age 19.7 years, age range = 18-28; 16 females, 6 males) completed the study. The average time the 22 subjects had their tongue jewelry at baseline was 2 months (range = 0.25-5 months). From baseline to 14 months, a significantly greater increase in gingival recession was found on the test side compared to the control side (P0.0001). In particular, the lingual surface of tooth #24 had a significantly greater increase in gingival recession compared to the facial surface (0.46 mm vs. 0.00 mm, p=0.00l). Tooth #25 also exhibited an increase in gingival recession on the lingual surface compared to the facial surface (0.38 mm vs. 0.00 mm); the difference approached statistical significance (p=0.09).
