Excerpt from Diphtheria: Its Cause, Nature, and Treatment In 1862, while engaged in an extended series of investigations of other pathological and aetiological questions, I fell upon evidences, satisfactory to my own mind, that the cause and nature of diphtheria differed most essentially, in many respects, from the teachings of the profession then, or, indeed, at any time since; and which, if true, demanded a radical Change of treat ment from what prevailed in either school then, or since, to better correspond with what appeared to be the truer nature of the disease. During the three succeeding years, however, or from 1862 to 1865, and until I had secured all the evidence then to be obtained to sustain the new path ological and aetiological views, I did not feel justified in changing the method of treatment I had been taught and previously followed quite successfully in this malady. That method excluded local treatment of any and all kinds, and was in other respects what would generally be considered strictly Homoeopathic; but, aside from avoiding topical applications, it was not the practice taught by Hahnemann, which so many, even of our own school, affect to despise, or. Excerpt from Diphtheria The excellence of the series of monographs issued under the editorship of Professor Nothnagel has been recognized by all who are sufficiently familiar with German to read these works, and the series has found a not inconsiderable proportion of its distribution in this and other English-speaking countries. I have so often heard regret expressed by those whose lack of familiarity with German kept these works beyond their reach, that I was glad of the opportunity to assist in the bringing out of an English edition. It was especially gratifying to find that the prominent specialists who were invited to co-operate by editing separate volumes were as interested as myself in the matter of publication of an English edition. These editors have been requested to make such additions to the original articles as seem necessary to them to bring the articles fully up to date and at the same time to adapt them thoroughly to the American or English reader. The names of the editors alone suffice to assure the profession that in the additions there will be preserved the same high standard of excellence that has been so conspicuous a feature in the original German articles. In the present volume it has been necessary to substitute for one of the German articles (that on "Diphtheria") an article by an American writer, owing to an arrangement made by the German author to issue a translation of his article apart from this series. While I regretted the necessity of this change very much, I was gratified in securing the acceptance of the task of writing an entirely new article by Dr. William P. Northrup, and the readers of the volume will feel that the substitution has entailed no lass to them. With the exception of the article on "Diphtheria," the articles in this volume and in the remaining volumes of the series will be those of the original German edition. Hence it might happen that one investigator studying one class of cases - for instance, the pseudo-membranous anginas associated with scarlet fever, measles, etc. - might obtain results different from another who observed another class of cases. In view of this uncertainty, it is clear that conclusions as to the causation of primary diphtheria should be derived from the investigation of cases which all are agreed in regard ing as genuine primary diphtheria. When the etiology of this undoubted class of cases has been determined, we are in a position to examine whether the secondary and other doubtful forms of diphtheria have a different causation or not. H., June, 1880 For the last sixty years, medical literature has been teeming, now and then, with descriptions of the terrible devastations which the disease has made throughout the civilized world. Its virulence has not abated in the least, but, rather, since 1858, seems to gather new strength as it moves from one place to another. At the pres ent moment, - autumn of I879, in Russia its fatality is terrible. In many portions of that country, it has attained such frightful pro portions that the percentage of mortality exceeds that of births. In some small districts, from one half to seventy per cent. Of the infant population have died. In the hamlet of Nakomobka, two hundred children succumbed to the disease in about ten months, in addition to a large number of adults.
