The process of freezing and storing red blood cells has been developed into a relatively simple and practical technic. The storage of frozen red blood cells has been approved by the FDA for up to 10 years of storage time. However once the blood is thawed and reconstituted, the red cells can be stored in a normal blood bank refrigerator for a period of only 72 hrs after which they must be discarded. This short storage period creates great problems in the military environment as well as in civilian life. The reasons for this short period of storage are usually stated as follows: A. The thawed red cells have been suspended in a saline preservative medium with a small amount of added glucose which has limited nutritive value. Over a period of time the red cells deplete the nutrients and begin to autolyze; B. Some of the storage media used for longer term preservation are not isoosmotic with red cells and thus produce some destruction of these cells due to hypotonic shock when they are placed in this medium during the storage period; C. As the red cells metabolize the glucose, the pH slowly drops. This decrease in pH causes a decreased viability of the red cells and decrease in the intermediary metabolites necessary for Rbc function; D. Mos of the methods of washing cells employ equipment that is not completely sealed and therefore there is a possibility that bacteria could be introduced into the blood and grow during the storage period. The purpose of this study was to determine if adding one of several currently acceptable anticoagulants to the blood after washing would increase their survival during storage. (ttl).

The FDA has approved the protocol by which red blood cells can be frozen with 40% W/V glycerol, stored at -80 deg C, deglycerolized using the Naval Blood Research Laboratory (NBRL) Standard Operating Procedure for non-rejuvenated red blood cells, and stored in a 0.9% sodium chloride-0.2% glucose solution for 24 hours at 4 deg C. The Department of Defense (DoD) has approved this method for the deployment of frozen blood banks where glycerol-frozen red blood cells are stored at -8 deg C for 10 years. The NBRL has collected the enclosed data to support the extension of the postthaw storage period from 24 hours to 5 days. We are requesting the extension of the post-thaw-wash storage period for red blood cells from 24 hours to 5 days in view of the fact that the FDA has approved the storage of platelets at 22 deg C for 5 days, and the potential for contamination is similar for both.

Method using oxygen removal for extending the useful shelf-life of refrigerated red blood cells. A cost-effective, 4.degree. C. storage procedure that preserves red cell quality and prolongs post-transfusion in vivo survival is described. Preservation of adenosine triphosphate levels and reduction in hemolysis and in membrane vesicle production of red blood cells stored at 4.degree. C. for prolonged periods of time is achieved by removing oxygen therefrom at the time of storage; in particular, by flushing with an inert gas. Adenosine triphosphate levels of the stored red blood cells are boosted in some samples by addition of ammonium phosphate.

Human CPD-collected red blood cells from 7 healthy male volunteers were stored in ADSOL (adenine, glucose, mannitol, and sodium chloride solution) at 4C for various periods: the red blood cells from one volunteer were stored for 35 days; red blood cells from 3 volunteers were stored for 42 days; and red blood cells from 3 other volunteers were stored for 49 days. A 100 ml aliquot was removed from each unit for in vitro and in vivo survival measurements. The remainder of the unit was biochemically modified with pyruvate, inosine, phosphate, and adenine (PIPA) solution, frozen with 40% W/V glycerol using a double centrifugation procedure and stored at -80C for 84 to 309 days. After thawing and washing, the previously frozen red blood cells were stored at 4C for 24 hours prior to in vitro and in vivo testing. CPD-ADSOL red blood cells stored at 4C for up to 42 days, biochemically treated with PIPA solution, frozen, thawed, washed and stored at 4C for 24-hours exhibited a mean in vitro recovery value of 83%, a mean 24-hour posttransfusion survival value of 72%, improved oxygen transport function, and only minimal residual hemolysis. CPD-ADSOL red blood cells treated in an identical manner except for a 49-day liquid storage period had a mean in vitro recovery value of only 73% and a mean 24-hour posttransfusion survival value of only 61%, but ATP, DPG, P50 levels, and the level of residual hemolysis were satisfactory. Our data show that 42-days is the maximum period that red blood cells can be stored in CPD-ADSOL solution at 4C and have satisfactory in vitro recovery and in vivo survival results following biochemical treatment with PIPA solution, freezing, thawing, washing, and post-wash storage at 4C for 24-hours.

The 1975 Cryogenic Engineering Conference also marked the first time the meeting had been held outside the territorial limits of the United States. Based on the enthusiastic response of the attendees and the exemplary hospitality of the Canadian hosts, it certainly will not be the last meeting to convene beyond the confines of the fifty states. The Cryogenic Engineering Conference Board is extremely grateful to The Royal Military College of Canada and Queen's University for the invitation to hold this meeting in Kingston, Ontario, Canada. This is a Standard Operating Procedure (SOP) to glycerolize and deglycerolize human red cells in the 800 ml primary polyvinylchloride (PVC) plastic collection bag system. The glycerolized red cells are concentrated and the supernatant glycerol removed prior to freezing. The glycerolized red cells are washed in the Haemonetics Blood Processor 115. Highlights of this revised SOP include a sterile docking procedure for glycerolizing the red blood cells, a change in centrifugation speeds, and new procedures for thawing the red blood cells prior to deglycerolization. Freezing in the primary PVC collection bag reduces the potential for contamination, eliminates the need for a special freezing bag, reduces the volume of wash solution, permits biochemical modification of indated and outdated red cells, increases the storage capability of the -80 C mechanical freezer, and reduces the cost of freezing a unit of red cells by 25%.
