This integrative review focuses on the current body of evidence for charge nurse leadership development (CNLD). It identifies common skills and competencies, provides a synthesis of existing training practices, and identifies gaps in the evidence. A thorough literature search of electronic databases and reference lists was completed, resulting in the identification of 22 studies published between 2011 and 2020. Data from the selected studies were evaluated and categorized into common themes; virtually all reported positive outcomes for the charge nurse development program implemented. Two of the studies reviewed were integrative reviews that compared identified themes and methodologies. Four studies were qualitative, identifying needed charge nurse competencies. Fifteen studies were quantitative, evaluating the effectiveness of charge nurse professional development programs implemented in the health care organization. One correlational study compared face-to-face learning with a blended learning format of face-to-face and online learning. Four major themes were identified: management of daily clinical operations, mentoring and empowering of staff, communication and conflict resolution skills, and role clarity and self-confidence. Two minor themes, budgetary knowledge and customer relations associated with patient/family-centered care, were identified. The majority of the charge nurse professional development programs were offered using multiple educational formats. Research supports the importance of a formal charge nurse professional development program to strengthen charge nurse role development and promote sustainable organizational goals and optimal patient outcomes.

Nurses and other clinicians are promoted to their first manager roles due to expert clinical performance; however, nurses and other clinicians also require nonclinical skills to succeed as a new manager. Many health care organizations (HCOs) are unprepared to develop clinicians as effective leaders. Across the United States, fewer than 30% of HCOs have formal leadership development programs. This study explores an evidence-based nurse leadership training program based on the American Organization for Nursing Leadership (AONL) Nurse Manager Competencies. The impact on the institution's return on investment--specifically, on improving new managers' perceived competency and acumen to effectively run their departments--was supposed to be measured with the Nurse Manager Skill Inventory Tool (NMSIT). However, due to the COVID-19 pandemic, I was only able to collect pretest and demographic information from the participants. I was also not able to complete my goal of using a repeated measures design to evaluate new managers who were given an online educational intervention using the University of Washington's Percipio platform, though the participants were all enrolled to the program.

Background: Nurses are commonly promoted to the role of nurse manager based on clinical competency rather than managerial skill or operational knowledge. The clinical nurse stepping into a managerial role may take up to seven years to develop the competency required to confidently perform in their new role. The transition from clinician to leader and manager, requires a structured development approach. Objectives: Leadership development for the nurse manager has a long-term impact with positive results within six to nine months after training. The nurse managers left to seek the development of their own competency are likely to find workarounds, but nurse managers offered formal leadership development are more likely to implement a culture of evidence-based practice and improved outcomes. Methods: Fourteen nurse managers, with less than seven years of experience in a managerial role within an acute care hospital setting, completed a nine-week structured development program. A pre- and post-competency assessment was conducted using the Nurse Manager Skills Inventory Tool to measure self-assessed knowledge in three domains: the science of managing the business, the art of leading people, and the leader within. Results: Thirteen out of fourteen participants completed the nine-week intervention program. Twelve out of thirteen submitted a pre- and post- competency self-assessment. The results reflected an improved mean score of self-assessed competency in all three domains. Domain 1: The science of managing the business had the greatest growth with a pre-intervention mean score of 1.78 0.83 to a 2.45 0.75 post-intervention. Domain 3: The leader within reflected the least growth with a pre-intervention mean score of 2.34 0.77 and a post-intervention mean score of 2.69 0.65. Literature suggests there is a link between nursing leadership training and decreases in patient mortality, medication errors, restraint use, patient falls with injury, and hospital acquired infections. In an effort to prepare charge nurses (CNs) tasked with leading unit based initiatives to improve patient safety, 24 competencies were identified and categorized based on the five domains as identified by the American Organization of Nurse Executive (AONE).
