Excerpt from The Medical News, Vol. 57: A Weekly Medical Journal; July-December, 1890 In passing from this merely statistical return to the peculiarities of the pneumonias of the past season, I would first note the large preponderance of cases affect ing the right lung. I should be inclined to say that this side was affected at least twice as often as the left. There was also an unusual proportion of cases of apex pneumonia where the entire upper lobe was completely consolidated without implication of any other part. As usual in such cases, there seemed to be a special pre dominance Of cerebral symptoms. It would appear that the association with pleurisy was even more constant and marked than usual. The side pain was often intense and sudden. The pleurisies were usually plastic; in one case, coming on with pneu monia immediately after exposure during convalescence, aspiration secured a pint and a half of pus on the third dav and a few ounces of sero-pus from the pericardium on the following day. Excerpt: ...ofVirchow conclusively showed that arterial and venous obstruction may be brought about by embolism and thrombosis independently of inflammatory change in the vessels. In fact, the changes in the walls of the vessels wrought by inflammation, such as roughening and thickening, which were thought to be especially causative of coagula, may, it is known, exist to a very great extent without producing any such obstructions. It is, for example, not uncommon to find the intima of the aorta and other vessels excessively roughened even by extensive calcification, without the occurrence of clotting. Into the minute pathology of this subject, which is still under discussion and needs much elucidation, I do not now propose to enter. My object is to call attention to a particular form of venous obstruction that occurs as a complication of typhoid fever, and which cannot be regarded as very uncommon, and yet relatively to many other complications of this disease is perhaps infrequent. The seat of the obstruction in the cases referred to is the femoral vein. The thrombus occasioning it is termed marantic or marasmatic, and in accordance with the meaning of this term it is believed by some to be due to feebleness of the circulation resulting from heart-weakness and consequent retardation of the blood-current. Whether such slowing and enfeebling of the circulation are of themselves sufficient to produce the thrombus, is, I think, at least open to question. What share the accumulation of blood-plaques or hasmatoblasts may have in its production, as they have been shown to have in the case of other thrombi, has not yet been clearly shown. The dried and pulverized cake may be gently warmed with sodium hydrate, in order to dissolve the lead chromate, without going so far as to decompose the albumen present. Either sodi sulphide or ammonium sulphide may now be added, and lead, if present, will be detected by a brownish or blackish coloration of lead sulphide. By either of these tests one one-hundredth of one per cent. Of lead chromate may be detected in the dried and pulverized cake. Only in cakes obtained from four out of the twenty-six shops visited was any lead found. In all four of these places lead was found in the tea buns purchased, and in the bright yellow pound cakes obtained in three of these places it was also found. The tea - buns were pale yellow in color, and although special tests for chromium were not applied, it is to be taken for granted that the lead was pres ent as the chromate, inasmuch as no incentive exists for the baker to introduce lead in any other form.
