Excerpt from The Ophthalmic Record, 1892, Vol. 2 In my investigations I have always considered the eye before which no prism is held as the one under test. With the double prism before the right eye the patient is asked about the position and the direction of the middle line. It may be nearer the bottom, thus showing left hyperphoria; or again, it may extend farther to the right than the other two and not so far to the left, thus showing exophoria, or vice versa, showing esophoria. Excerpt from The Ophthalmic Record, Vol. 2: December, 1892 Throughout the long controversy which has existed between the specialists and those in authority, the one question invariably put by the latter, has been, - point out cases which have been proved to be due to color blindness or defective sight? For a long time this question could not be answered, because sailors on colliding vessels are not required to have their eye-sight tested, but we fortunately can now point to three cases where color-blindness or defective-sight was the sole cause oi disaster. The left eye was always myopic to a high degree, but the patient was able to follow his occupation, book-binder, until April 13, 1880, when he suddenly went blind. He could not give a clear history Of the cause, other than the fact that the sight grew bad and the cause was attributed to a cataract. Excerpt from The Ophthalmic Record, Vol. 3 Heymann was the first to call attention to the retinal changes, which he regarded as characteristic in interstitial nephritis. Every Ophthalmologist can readily call to mind cases in which the Ophthalmoscope first revealed the existence Of albuminuric retinitis, the diagnosis being subsequently confirmed by a urinary analysis. Within the last week a patient presented himself at the Eye Clinic for relief Of failing vision. As is usually the case with these patients, she had first consulted the itinerant spectacle vendor for glasses, and, finding no betterment Of her vision, she applied to us for relief. The Ophthalmoscope re vealed an extensive retinitis with extensive retinal hemorrhage. Examination of the urine showed about 40 percent. Albumin by quantity. This woman, according to her history, did not com plain Of untoward symptoms Of any kind, merely a failing Of her vision. Excerpt from The Ophthalmic Record, Vol. 7: December, 1898 To see a thing you must get within seeing distance of it. A certain area cannot be perceived beyond a certain distance. The half-metre test types are very useful at the distance of a half metre or less, but they are of very little use at one metre, and quite worthless at two, four or six metres. In skiascopy there are movements of light and shadow in the pupil that can be seen at one-half metre, which are not to be seen at one metre or any greater distance and in many eyes it is these movements, and these alone, that truly indicate the refraction of the eye. Excerpt from The Ophthalmic Record, Vol. 8: September, 1899 The ocular condition may go long undetected, for subjective symp toms may, and generally do, remain absent until the case is very far advanced indeed. So long as the macula escapes and the disordered areas are individually of small extent the eye may be yet very seriously diseased before the vision is notably affected. The state of the eye is not infrequently quite unexpectedly discovered, which is often the patient's as well as the physician's first indication of any serious trouble. When vision does become affected it often rapidly goes down hill, though it seldom entirely disappears. The affection is generally binocular, though a number of monocular cases have been reported. I have myself seen only one such. This is mainly a disease'of late middle life, but cases are found in youth, even in childhood. These latter seem rather to indicate a dependence upon congenital syphilis. It is not really known in what proportion of cases of granular kidney albuminuria develops, though various statements have been'made. The ophthalmosc0pic' picture of typical retinitis albuminurica ought to be of interest to all_practitioners of medicine, because it reveals to the eye in the living body pathological changes which may be more marked in the retina than in other tissues, its vascular supply being terminal, without anastomoses, but is illustrative of lesions present in other and more vital organs, such as brain and kidney. These visible ocular changes have been best described by Mar cus Gunn, but I shall place them under five heads. One or other of them usually predominates, though they may all be present at the same time. They are: (i) vascular changes, (2) hemorrhages, (3) white spots, (4) exudations, (5) neuritis. Excerpt from The Ophthalmic Record, Vol. 4: February 1, 1895 Knapp, in replying, said that it was on the strength Of his recent experience that he maintained that the simple operation is not only the safest, but the best Operation. Swanzy credited him with 10 per cent. Of prolapses, but Only fourteen cases, or per cent. Of all, had required Operative treatment. The oth ers recovered spontaneously, some with persistence of a small un irritative protrusion, not seriously interfering with sight, and in the others the prolapse disappeared. The eye, after a combined extraction, was, by the frequent incarceration Of invisible iris and capsule in the scar, less resistant to Obnoxious influences than when the iris was preserved. He practiced secondary dis cission in about two thirds Of his cases, and few patients Oh jected to it. If successful, it left an ideal eye. The discission can be done at any time, without notable reaction, if the pupil is free from inflammatory products.
