Second only to the common cold, pain is the most common reason for health care visits. It results in significant personal and social costs to individuals, and economic costs to society. Chronic pain has been estimated to cost more than 100 billion annually in the United States, and its estimated annual cost to European countries ranges from 1.1 to nearly 50 billion Euros (nearly 1.5 billion to over 60 billion dollars). In order to treat pain, clinicians need to be able to assess pain and its effects on functioning. However, 100s, if not 1000s, of different measures of pain and pain-related domains exist. Surprisingly, despite the fact that much is known about the psychometrics strengths and weaknesses of existing measures, there does not yet exist a simple pain assessment guide that describes the most useful and psychometrically sound pain measures for health care providers. This book fills this need.

Pain and how to measure pain has always been a challenge to the profession of nursing. The best way to assess patients' pain is by having them self-report or verbally inform the healthcare provider by evaluating how the pain feels. The American Pain Society calls the self-report of pain the "Gold Standard of Pain Assessment." The struggle as to how to measure pain occurs when a patient is unable to verbally explain to the nurse that they are in pain/the severity of their pain or if there is a new type of pain they are experiencing. The Nonverbal/Unable to Self-Rate Pain Scale is a tool utilized to help rate pain nonverbally for those who are unable to communicate pain. This tool is a combination of three different nonverbal pain scales which had not been shown to have a relationship with a verbal pain score. This quantitative pilot study compared post-operative patients' reported verbal pain score with the score the principal investigator observed, utilizing the Nonverbal/Unable to Self-Rate Pain Scale. The sample was a convenience sample. The study was conducted at a suburban hospital containing 150 beds. The unit participating in the study was a 47 bed medical/surgical floor specializing in bariatric surgery. The data were analyzed utilizing simple and descriptive statistics. The theoretical framework utilized for this study was Kolcaba's Comfort Theory. The outcome of this study revealed that a nonverbal observed pain score was consistently lower than the verbal pain scores reported by post-operative patients. The clinical implications are the Nonverbal/Unable to Self-Rate Pain Scale is not a useful tool in measuring pain. However, the study did find that the Nonverbal/Unable to Self-Rate Pain Scale was more accurate with the male population. Further input with a broader patient scope could further and better evaluate the effectiveness of the pain scale utilized.

Background: Pain is one of the most common symptom reported in general practice. For this purpose, one-dimensional and multidimensional pain scales have been developed to assess acute or chronic pain. There is no gold standard in pain assessment in general practice. This study aimed to list all pain scales studied for the general population. Method: A systematic review questioning Cochrane database, according to the PRISMA protocol, was conducted until February 2018. Only English articles assessing pain scales in general population were included. The bibliography of selected studies and recent systematics reviews had been examined to include additional articles in a second search. Results: On 1431 results found on Cochrane, 4 relevant articles were included. With the second search, 203 additional articles were detected and 84 were added. Finally, 88 articles were selected and 35 pain scales were found: 6 unidimensional-scales with 13 variants and 12 multidimensional scales with 4 variants. Discussion: Pain scales were different by explored dimensions and the number of item. Some of them are not studied in this current review by a lack of articles available on the database responding to eligibility criteria. Reliability data of available pain assessment scales were recently collected in a systematic review. It will be necessary to consider external validity data in further studies and to choose which pain scale is the most appropriate for a daily practice.

Pain resulting from physical trauma and surgical procedures is very strong. In elderly patients, especially those with impaired communication or cognitive damage, it is assessed rarely or not at all. The importance of pain assessment is reflected in quality supervision and adequate treatment which decrease complications and speed up recovery. The aim of the study was to determine the metric characteristics (reliability and validity) of the Abbey Pain Scale on a population of elderly patients with impaired communication in Croatia and to demonstrate the correlation between the assessed acute pain level and analgesia efficacy.The sample consisted of 31 patients above the age of 65 hospitalized after a femur fracture atthe Trauma Department of University Hospital Centre u201cSestreMilosrdniceu201d in Zagreb, Croatia. The Abbey Pain Scale, developed by professor Jennifer Abbey at Queensland University in Australiawas used for pain assessment. The patientsu2019 mental status was evaluated using Mini-Mental State Examination (MMSE). The reliability of the Abbey Pain Scale was confirmed by Cronbachu2019s alpha coefficient of internal consistency whose value was acceptable at 0.561.For the purpose of determining the validity of the Abbey Pain Scale, the results of this assessment were correlated with patients' self-assessment using Visual Analogue Scale (VAS).The correlation of pain assessment after analgesia was high and significant, which leads to the conclusion that the Abbey Pain Scale is a convenient tool for assessing pain intensity in patients with impaired communication due to good metric characteristics of validity and reliability. This study has demonstrated its effectiveness in assessing pain, which was indirectly related to satisfactory analgesia.
